Application Of OSCE In The Evaluation Of Standardized Training Nurses' Clinical Practice Ability

ObjectTo develop a scientific and effective standardized training nurses' OSCE assessment program,and to assess the nurses who completed two years of standardized training in a tertiary-grade A class hospital in Qingdao.Then exploring the feasibility of this program,evaluating the clinical practice ability of standardized training nurses,observing and analyzing the application effect.Method1.Review the literature,preliminary drafting the OSCE assessment program for standardized training nurses,and use the Delphi method to inquire about 20 experts in related fields.The contents include the setting of the test station and the assessment contents,the examination process arrangement,and the personnel training.When the consistency of the experts is high,the inquiry is stopped.Projects with an importance value above 3.5 points and a coefficient of variation below 0.25 were included in the assessment content,and the examination process was optimized according to expert opinions,and the OSCE assessment plan for standardized training nurses was finally established.2.Using the OSCE assessment program of standardized training nurses,to evaluate 162 nurses who completed two years of standardized training in a tertiary-grade A class hospital in Qingdao.There are 5 stations in the examination,which are the nursing evaluation station,the clinical thinking station,the basic nursing skill station,the emergency nursing skill station and the humanistic nursing communication station.In order to save the test time,two sets of parallel test stations were set up.There are two examiners at each test station.In the pre-test preparation stage,preparing standardized test cases,and developing a unified scoring standard,which will be reviewed and revised by clinical nursing experts,4 standardized patients and 15 workers were recruited from nursing graduate students,20 examiners were recruited from the hospital nursing management staff,and they were separately trained.According to the examination venue,the distribution of examination stations and examination time are reasonably arranged.Before the test begins,the candidates will determine the group by lottery.The off-site staff will explain the test process to the candidates.The candidates who complete the test will leave the test room after completing the questionnaire.After the exam,collecting the score data,conducting statistical analysis,using the difficulty,discrimination,reliability and validity indicators to analyze the quality of the assessment program,and obtain the feedback of the candidates through the questionnaire survey results,and explore the feasibility and application effect of the assessment program.Results1.After two rounds of enquiries,the opinions of experts tend to be unified,and the letter of inquiry was stopped.19 experts from 12 t tertiary-grade A class hospitals and universities in 9 cities across the country have completed two rounds of effective enquiries.The consultation established five test stations,such as nursing assessment,clinical thinking,basic nursing skills,emergency nursing skills,and humanistic nursing communication,as well as specific assessment contents and process settings.The positive coefficients of the two rounds of experts were 100%and 95%respectively;the expert authority coefficient was 0.846,0.851 respectively;the first round of the study asked Kendall's harmony coefficient to be 0.292,?2 to be 286.489,and the degree of freedom to be 49,P<0.01.In the second round,Kendall's harmony coefficient was 0.373,?2 was 326.082,and the degree of freedom was 46,P<0.01.2.Using the OSCE assessment program to evaluate the clinical practice ability of standardized training nurses.The average scores of the five test stations were 75.94±7.75,79.51 ±6.41,80.24±5.83,80.37±7.56.75.17±7.28 points,the difficulty were 0.75?0.80,the discrimination were 0.45?0.61,and the Cronbach's a was 0.783.According to the survey results,21.4%of the candidates knew about OSCE before the test,and 45.12%of the candidates thought that the performance of the human nursing communication test station was not good,but the test can test their communication skills and learn communication skills.78.3%of the candidates believe that the overall difficulty of the examination is moderate,the assessment method is novel and objective,and it helps to improve the clinical practice ability.Conclusions1.This study has established a standardized training nurse OSCE assessment program established by Delphi method,which is scientific and reliable,and provides a basis for the evaluation of standardized clinical training ability of training nurses.2.The OSCE assessment program of standardized training nurses developed in this study has high reliability,good content validity,moderate difficulty in trial questions,and moderate degree of discrimination.It has certain feasibility and is suitable for the evaluation of clinical practice ability of standardized training nurses.It can objectively evaluate the clinical practice of nurses.Practical ability,the effect is better.It can help nurses discover the advantages and disadvantages of clinical practice ability,clarify goals,improve them,and provide training plans for nursing managers to improve the overall quality of nursing teams.3.This study also has certain limitations,such as less sample capacity;standardized patients recruited from students,it is difficult to fully express some of the positive signs of the disease.The next step of research can expand the sample capacity,expand the scope of recruitment of standardized patients,and improve the quality of assessment.
