Thyroid Function In Patients With Type 2 Diabetes And Their Influencing Factors

Abstract/Summary:
Objective: To study the clinical data of patients with type 2 diabetes mellitus(T2DM),to analyze the prevalence of thyroid dysfunction and thyroid autoantibodies in patients with T2DM,and to analyze the gender differences;to explore patients with thyroid dysfunction and thyroid autoantibodies.Clinical characteristics,and analysis of the influencing factors of T2DM with thyroid dysfunction,in order to explore the relationship between T2DM and thyroid dysfunction,provide evidence for clinical diagnosis and therapy,Intervene as early as possible if necessary.Methods: 642 patients who were admitted to the Department of Endocrinology of Hunan Provincial People’s Hospital from January 2017 to December 2018 were selected as subjects with better type 2 diabetes.Record basic patient information including gender,age,height,weight,BMI,duration of diabetes,type of hypoglycemic drugs;and record laboratory indicators of patients including fasting blood glucose(FPG),glycosylated hemoglobin(HbAlc),fasting C-peptide(FCP),postprandial2 h C-peptide(PCP),triglyceride(TG),serum total cholesterol(TC),high-density lipoprotein(HDL),low-density lipoprotein(LDL),free triiodothyronine(FT3),free thyroxine(FT4),thyroid stimulating hormone(TSH).Results:(1)Among 642 patients with type 2 diabetes,108 cases of thyroid dysfunction occurred,accounting for 16.82%;49 cases of subclinical hypothyroidism occurred,accounting for 7.63%,accounting for 45.37% of type 2 diabetes with thyroid dysfunction;clinicalhypothyroidism occurred Thirteen cases,accounting for 2.02%;4 cases of subclinical hyperthyroidism,accounting for 0.62%;5 cases of clinical hyperthyroidism,accounting for 0.78%;37 cases of low T3 syndrome,accounting for 5.76%.Among them,218 patients were screened for thyroid autoimmune antibodies,17.61% were positive for TGAb and TPOAb,10.93% were positive for single antibody TGAb,and 6.93%were TPOAb positive for single antibody.(2)In this study,642 patients with type 2 diabetes mellitus were treated with normal levels of thyroid function and thyroid function group[hypothyroidism group(subclinical hypothyroidism and clinical hypothyroidism),hyperthyroidism group(subclinical hyperthyroidism and clinical hyperthyroidism),low In the T3 syndrome group,the clinical data of each group were compared.Compared with the normal group,the hypothyroidism group had higher FPG,HbAlc,FCP,PCP,HOMA-IR(CP),TG,and TC.In the normal group of Jiagong,the difference was statistically significant.(P<0.05)(3)Multivariate logistic regression analysis showed that female,elderly,FPG,HbAlc,HOMA-IR(CP),TG were risk factors for hypothyroidism in type 2 diabetes mellitus patients,and metformin was protective factor for hypothyroidism in type 2 diabetes mellitus patients.Conclusion:(1)Type 2 diabetes patients with thyroid dysfunction are more common in the clinic,accounting for up to 16.82%,with the most common subclinical hypothyroidism.(2)In patients with type 2 diabetes,blood glucose and blood lipids were significantly higher than those with normal thyroid function.(3)Women,advanced age,FPG,HbAlc,HOMA-IR(CP),and TG are risk factors for hypothyroidism in patients with T2DM.Treatment of patients with T2DM with metformin is a protective factor for hypothyroidism.
