| OBJECTIVE:To make a comparison on the clinical curative effect of respectively using autologous blood vessels and artificial blood vessels as materials to do femoral vein encircling constriction to cure primary deep venous insufficiency.MATERIALS AND METHODS:102 patients with primary deep venous insufficiency hospitalized in 163 Hospital of People’s Liberation Army from February 2012 to February 2017 were divided into autologous blood vessel group(group A)and artificial blood vessel(group B)according to the surgical procedures, including 60 patients in group A and42 patients in group B.Patients in group A conducted high ligation of great saphenous vein+ segmentation stripping operation and also used autologous great saphenous vein to implement femoral vein encircling constriction.Patients in group B accepted high ligation of great saphenous vein+ segmentation stripping operation or had poor conditions of great saphenous vein.Artificial blood vessels were used to implement femoral vein encircling constriction. The curative effect was evaluated by making a comparison their clinical curative effect score and using color ultrasound to check blood backflow.Meanwhile, through five-year follow-up,patients’ relapse situation was evaluated and compared.RESULTS:Clinical symptoms of 102 patients in group A and group B were relieved.Blood backflow situation turned better.Patients in group A had lymphatic fistula in four limbs, but there was no wound infection.Patients in group B had lymphatic fistula in three limbs and one limb hadwound infection. There was no statistical difference in postoperative complications and length of stay between group A and group B(c12=0.005 P1=0.941,c12=1.420 P2=0.233).Length of stay in group A was 13.57±2.31 d and that of group B was 13.50±2.74 d.There was no statistical difference in length of stay between group A and group B(t=0.133 P=0.895).Color ultrasound was used to evaluate postoperative backflow of patients. There was no statistical difference in the curative effect of both groups(Z=-0.446,P=0.662 t=0.133 P=0.895).Preoperative clinical score of group A was 5.77±1.23 and postoperative score was1.57±1.29.Preoperative clinical score of group B was 5.92±1.19 and postoperative score was 1.43±1.2.There was the statistical difference in clinical score before and after the operation(t=-0.664 P=0.508).The score difference of both groups before and after operation was respectively marked as 4.26±1.07 and 4.61±1.12.There was no statistical difference(t=-1.731 P=0.086).CONCLUSIONS:There is no statistical difference in postoperative complications and length of stay by using autologous blood vessels and artificial blood vessels as materials to do femoral vein encircling constriction to cure primary deep venous insufficiency. Both of them can obviously improve symptoms and effective control blood backflow. Both materials have the similar curative effect. |
