Detection Of Combined Detection Of RF,Anti-Carp Antibody,Anti-CCP Antibody,Anti-MCV Antibody And Anti-hnRNPA2 Antibody In Patients With Rheumatoid Arthritis

ObjectiveTo investigate the serum rheumatoid factor(RF),anti-carbamylation protein(Carp)antibody,anti-cyclic citrullinated peptide(CCP)antibody,anti-mutant type in patients with rheumatoid arthritis The detection value of citrate citrullinated vimentin(MCV)antibody and heterogeneous nuclear ribonucleoprotein A2(hnRNP A2)antibody against RA.MethodsA total of 97 RA patients admitted to Tianjin Hospital,Tianjin Medical University General Hospital Aviation Hospital and Tianjin Fifth Central Hospital from March 20 to 2019 were selected as RA group.Seventy patients with non-RA immune system disease were selected at the same time.In the non-RA group,93 healthy subjects were selected as the control group,and peripheral blood samples from each group were collected for testing.1.RF detection: The serum RF value of the experimental population was detected by Beckman's RF test kit.2.Anti-CCP antibody detection: The anti-CCP antibody in the experimental population was tested by ELISA using the synthetic biotinylated CCP polypeptide as the antigen.3.Anti-Carp antibody detection: ELISA was used to detect anti-Carp antibodies in the experimental population by using carbamylated fetal bovine serum(prepared by the laboratory)as an antigen.4.Detection of anti-hnRNPA2 antibody: The E.coli expression system was selected to clone,express and purify hnRNPA2.ELISA was performed on anti-hnRNPA2 antibodies in the experimental population using hnRNPA2 as an antigen.5.Anti-MCV antibody detection: Select E.coli expression system,clone,express and purify mutant vimentin,and citrify the mutant vimentin to prepare citrullinated mutant vimentin as detection antigen.Anti-MCV antibodies were detected in the human by ELISA.The sensitivity,specificity and Yoden index of RF,anti-Carp antibody,anti-CCP antibody,anti-MCV antibody,anti-hnRNP A2 antibody,single detection and combined detection were compared,the receiver operating characteristic curve was drawn,and the area under the curve was calculated.Results1.The mutant vimentin and hnRNP A2 were successfully amplified by E.coli system.The mutant vimentin and hnRNP A2 were purified and purified.2.Carbamate was used to carry out carbamylation of fetal calf serum to prepare carbamate fetal calf serum.3.The citrullinated mutant vimentin was prepared by citrullination of the expressed mutant vimentin using PAD4.4.The positive rates of RF,anti-CCP,anti-Carp,anti-MCV and anti-hnRNPA2 antibodies in RA group were higher than those in non-RA disease group and control group,the difference was statistically significant(p<0.05).5.The sensitivity was 65.98%,the specificity was 74.85%,the Yoden index was0.408,and the AUC was 0.704.The sensitivity of the anti-CCP antibody to RA detection was 73.19%,the specificity was 85.89%,the Yoden index was 0.591,and the AUC was 0.795.The sensitivity of the anti-Carp antibody to RA detection was41.24%,the specificity was 85.89%,the Yoden index was 0.271,and the AUC was0.636.The sensitivity of the anti-MCV antibody to RA detection was 71.13%,the specificity was 87.73%,the Yoden index was 0.589,and the AUC was 0.794.The sensitivity of anti-hnRNPA2 antibody to RA detection was 30.93%,the specificity was 90.8%,the Yoden index was 0.217,and the AUC was 0.609.6.The combination of RF anti-CCP antibody combined detection,sensitivity was85.56%,specificity was 73.62%,The Yoden index is 0.592 and the AUC is 0.796.RF anti-CCP antibody The anti-Carp antibody combined detection sensitivity was90.72%,the specificity was 72.39%,the Yoden index was 0.631,and the AUC was0.821.RF anti-CCP antibody Anti-Carp antibody The anti-MCV antibody combined detection sensitivity was 95.88%,the specificity was 71.78%,the Yoden index was0.677,and the AUC was 0.845.RF anti-CCP antibody Anti-Carp antibody The combined detection sensitivity of anti-hnRNPA2 antibody was 92.78%,the specificity was 71.17%,the Yoden index was 0.640,and the AUC was 0.828.RF anti-CCP antibody anti-hnRNPA2 antibody The anti-MCV antibody combined detection sensitivity was 94.85%,the specificity was 71.78%,the Yoden index was 0.666,and the AUC was 0.840.When the five indicators were combined,the sensitivity was98.96%,the specificity was 70.55%,the Yoden index was 0.695,and the AUC was0.861.Conclusion1.The sensitivity of combined detection and diagnosis of RA was higher than that of each index(p<0.05),and the Yoden index and AUC value were higher than single detection.2.Combined detection of the Jordan index,AUC value: five indicators combined detection>RF anti-CCP antibody anti-Carp antibody anti-MCV antibody>RF anti-CCP antibody anti-MCV antibody anti-hnRNPA2 antibody>RF anti-CCP antibody anti-Carp antibody anti-hnRNPA2 antibody> RF anti-CCP antibody anti-Carp antibody > RF anti-CCP antibody.
