Objective:The aim of this study was to measure the internal carotid artery angle,common carotid artery bifurcation angle of the same individual and three kinds of carotid bifurcation form,analysis the correlation between the anatomy of carotid artery bifurcation and the composition of carotid atherosclerotic plaque.Methods:Patients with carotid artery stenosis received carotid MRI examination in Tianjin The fouth Hospital from September 2013 to September 2014 were included in this study.A total of 114 cases(228 blood),68 cases of men,46 cases of women,41 to85 years old,average 63±9.07 years old.Inclusion criteria: Have bilateral or unilateral carotid artery atherosclerotic plaque on the ultrasound screening;No carotid intima of peeling or carotid artery stent implantation;Without MR check contraindication;No obvious heart,liver,kidney insufficiency;Exclusion criteria: carotid artery is so circuity that cannot measure the carotid bifurcation angle on the image;Line carotid intima of peeling or carotid artery stent implantation;In a critical condition can't row MR examination or have other MR examination contraindications;Carotid MR image quality level is 2 or less.Use Philips Intera Achieva 3.0 T MRI scanners,8 channel neck surface of phased-array coil and pulse control,for all patients with bilateral carotid artery line3 D TOF bright blood,black blood T1 WI,T2WI and PDWI scanning,analysis of the components of patients with carotid plaques.Patients Group there are two ways: 1)according to the different between internal carotid artery angles and the common carotid artery bifurcation Angle on both side of the same individual is divided into two groups: Large Angle Group(LAG),Small Angle Group(SAG).2)according to the carotid bifurcation form is divided into straight bifurcate,straight bent and bent bifurcate three categories.Results:(1)In 114 cases,101 cases of patients(about 88.6% of the tested blood vessels)can display the carotid bifurcation structure and plaque composition,bifurcate Angle measurement can be performed.(2)The left internal carotid artery angle was 30.59 ±14.61 °,the right internal carotid artery anglewas 24.01±11.51°;The left common carotid artery bifurcation Angle was 48.53 ±16.30°,the right common carotid artery bifurcation Angle was 43.94 ±15.08°.The left internal carotid artery Angle of 20 ° ~40 ° is more,the right internal carotid artery Angle of 10 ° to 30 ° is more;The left carotid artery bifurcation Angle > 40 ° is more,the right carotid artery bifurcation Angle of 30 ° ~ 50 ° is more.(3)The rate for vulnerable plaques between LAG and SAGof internal carotid artery is(50.5%vs36.6 %,p<0.05),the difference has statistically significant;The rate of Lipid or necrotic coreof plaque composition is(50.5%vs36.3%,p<0.05),the difference has statistically significant.(4)The rate The rate of vulnerable plaque and plaque compositon had no statistical difference of each component in the carotid artery bifurcation group.(5)Vulnerable plaque and plaque compositon had no statistical difference in the three types of carotid arterybifurcation group.Conclusion:(1)High resolution MRI can clearly show that most subjects of carotid ifurcation structure and plaque composition,bifurcation Angle can be measured on the oblique-sagittal black blood sequence.(2)Significant variationsexist in anatomy of left and right carotid artery bifurcation angle and internal carotid artery agle.Embodies the diversity of the carotid artery bifurcation anatomical morphology.(3)Hemodynamics is closely related to the occurrence of atherosclerosis.The detection rate of vulnerable plaque and LRNCin the internal carotid artery LAG group is high.(4)The detection rate of vulnerable plaque and plaque compositon had no statistical difference in the carotid artery bifurcation group and the three types of carotid arterybifurcation group.
