Purpose: Through retrospective analysis of the imaging manifestations of unilateral benign sphenoid sinus lesions in CT and MRI,we compared the manifestations of the same benign sphenoid sinus lesions in CT and MRI,analyzed the different manifestations of the same lesions in CT and MRI,and discussed the clinical application value of CT and MRI in unilateral benign sphenoid sinus lesions.Method: 33 patients with unilateral benign sphenoid sinus lesions were examined by CT and MRI,and the CT and MRI manifestations of the same lesions were identified and diagnosed. The specific imaging manifestations of the same lesions in CT and MRI were analyzed. The different imaging manifestations of the same lesions in CT and MRI and their coincidence with clinical diagnosis were analyzed. All 33 patients were confirmed by endoscopic sinus surgery and postoperative pathology.Result: Among 33 cases of unilateral benign sphenoid sinus lesions,12 cases were cysts.CT manifestations included round or oval soft tissue density in the sphenoid sinus and thinning of bone absorption in the sphenoid sinus with swelling changes.MRI manifestations: irregular spherical signal shadows on long T1 and long T2;CT manifestations of fungal lesions in 10 cases: irregular patchy calcification or cloudy flocculent density enhancement in sphenoid sinus, showing ground glass changes.MRI manifestations: mixed signal in sphenoid sinus, high signal in inflammatory flake exudation, low signal in fungal nodules on T2WI;CT manifestations of cerebrospinal fluid rhinorrhea in 5 cases: simple soft tissue density shadow in sphenoid sinus, high signal shadow in cerebrospinal fluid and high signal liquid shadow in nasal sinus are connected by high signal shadow in line, and other tissues(brain tissue, nasal mucus)The CT findings of 6 cases of inverted papilloma were as follows: soft tissue density in sphenoid sinus was not uniform.MRI findings: the tumor was heterogeneous and showed stripe like or varying degrees of "gyrus".Compared with the detection rates of CT and MRI in sphenoid sinus cyst, fungal sphenoid sinusitis, inverted papilloma of sphenoid sinus and cerebrospinal fluid rhinorrhea of sphenoid sinus, there was no significant difference in the diagnosis of sphenoid sinus lesions between CT and MRI by chi-square test(P > 0.05).Conclusion: CT can accurately show the changes of bone around lesions, while MRI can clearly show the extent of lesions. The combination of CT and MRI can provide more comprehensive imaging information for the diagnosis and clinical treatment of unilateral benign sphenoid sinus lesions.CT combined with enhanced MRI is more valuable than CT alone in differential diagnosis;CT and MRI have a high diagnostic coincidence rate for unilateral sphenoid sinus lesions, and the coincidence rate with pathological diagnosis is also higher, which is worthy of clinical application.
