Part ? MR T2WI And MRV Study of Transverse Sinus Arachnoid Granulation.Objective To explore the diagnostic value of MR T2WI and CE-MRV for AG in transverse sinus.Methods Collected clinical and imaging data from our hospital from January 2016 to April 2019 due to headache,dizziness or physical examination and other MRI+ CE-MRV examination cases,a total of 811 cases were collected,T2WI and MRV were recorded separately.The number of cases of AG in the detected transverse sinus,the number,size,shape of AG and the relationship between the adjacent veins.Result The AG was found in 184 cases using T2WI sequence,123 cases were detected in the left transverse sinus,91 cases were detected in the right transverse sinus,and AG differences were detected in the left and right transverse sinuses?P = 0.001?;AG was found in 119 cases using MRV,82 cases were detected in the left transverse sinus,50 cases were detected in the right transverse sinus,and AG differences were detected in the left and right transverse sinuses?P <0.001?;the difference in the number of cases of AG detected by the two methods was statistically significant?P <0.001?.All AGs detected by MRV can be detected by T2WI,and AG detected by T2WI sequence can be divided into MRV detected group and MRV undetected group according to whether they can be detected by MRV.The diameter of AG was?5.30±2.10?mm,and the diameter of the detected group was?6.14±2.03?mm.The difference in AG diameter between the two groups was statistically significant?P = 0.002?.T2WI was used to find 231 round or elliptical AG and 11 lobulated AG,while MRV could only show round or oval filling defects,and no lobed AGs were seen.T2WI observed 289 veins adjacent to AG,71 Labbé veins,and 218 non-Labbé veins;MRV observed 97 veins adjacent to AG,56 Labbé veins,and 41 non-Labbé veins,It is statistically significant?P <0.001?.Conclusion The AG in the transverse sinus is basically adjacent to the vein.Compared with MRV,T2WI can find more cases of AG in the transverse sinus,and can find a smaller AG in the transverse sinus,better showing the shape of the AG and its adjacent vein.Part ? MRI Study About The Relationship Between Dural Sinus Arachnoid Granulation With Age Objective To study the relationship between the arachnoid granulation in dural sinus with age by MRI.Methods The clinical and imaging data of 360 cases with routine MRI and enhanced scans suggesting that there were no abnormalities in the brain were collected.Based on the WHO age classification criteria,this article divided the?020?years into juvenile groups,?2060?age group is youth group and over 60 years old is middle-aged and elderly group,120 cases in each group,each group is subdivided into 4 age groups.According to the development speed,it is divided into four stages with different time,each age group is 30 cases,a total of 360 cases.Observe the distribution of AG in the venous sinus,the size and number of AG in each age group,and the morphological changes of AG.Results MR examination revealed that 163 AG were the most common sites in the transverse sinus;followed by 116 superior sagittal sinuses,50 straight sinuses,and 11 sinus sinks;cavernous sinus was not seen;the juvenile group increased with age,The diameter of AG gradually increases,the number gradually increases,and the number of leafy AG also increases;the diameter and number of AG in the youth group fluctuate within a fixed range with age,the shape remains basically unchanged,and cases of AG can be detected Compared with the minor group,the number and diameter were statistically significant?P <0.001,P = 0.023?;In the middle-aged and older groups,with the increase of age,the diameter of AG decreased,the number decreased,and the number of lobulated AG also showed a decreasing trend,The number of cases that can detect AG is statistically significant compared with the diameter of the young group?P <0.001,P = 0.016?.Conclusion The diameter of AG in the juvenile group gradually increased,the number increased,and the number of lobulated AG also increased;the juvenile group remained basically unchanged,while the middle-aged and elderly groups showed a decreasing trend.These changes may be closely related to growth and development,aging,and the regulation of cerebrospinal fluid absorption.Part ? The MRI Study of the Anatomy of Labbé Veins with Arachnoid Granulation Objective To explore the anatomical relationship between Labbé Vein?VL?and AG through MR observation.Methods Collected clinical and imaging data of 40 cases with AG in the vicinity of Labbé veins detected by MRI + CE-MRV in our hospital,and import the CE-MRV initial data into the 3D vein image reconstruction system of the neuronavigator,and construct the 3D vein model.The reconstructed 3D vein image model observes the anatomical relationship between Labbé vein and AG.Results 3D vein image observation showed that in 40 cases,a total of 53 AGs were found near the Labbé vein,44 AG were located upstream of the Labbé vein entrance,6 were located downstream of the Labbé vein entrance,and 3 were outside the transverse sinus,which was not related to the Labbé vein entrance.Among them,20 AGs did not contact the end of Labbé vein;14 AG contacted the end of Labbé vein,but the end of Labbé vein did not change,such as twisting,stenosis and drainage direction change;19 AG caused Labbé vein twisting,stenosis and drainage direction Changes;no Labbé veins or other small veins entered the AG;the anatomical relationship between the Labbé veins and the AG was not related to the size of the AG.A total of 15 AGs were found on the straight sinus,14 appeared in the junction of the great cerebral vein and the straight sinus,1 AG appeared in the posterior third of the straight sinus,and the size was?3.88 ± 0.74?mm.No venous vessels were found in the filling defect.Conclusion Most of the AG occurs upstream of the Labbé vein entrance,and most of the AGs near the Labbé vein are closely related to it.Some AG cause the Labbé vein to be twisted,narrowed,and the direction of the drainage is changed.The reconstructed 3D vein image observation of the relationship between veins and AG shows that no Labbé veins and small veins enter the AG.Part ? MRI study of arachnoid granulation in patients with cerebral venous sinus thrombosis Objective To study the changes and differentiation diagnosis of arachnoid granulation in patients with cerebral venous sinus thrombosis.Methods The clinical and imaging data of 35 patients diagnosed as CVST by MRI combined with CE-MRV were collected and set as the thrombus group;35 healthy persons who were the same age as the thrombus group and showed no abnormalities in the brain by MR examination were randomly selected as the control group,Count the size and number of AG in two groups,and record and compare the MRI characteristics of AG and CVST patients in each stage.Result In the thrombosis group,14 cases of AG were found with a diameter of?5.19±2.40?mm;in the control group,23 cases of AG were found with a diameter of?4.45±2.03?mm.The difference in the number of AG found between the two groups was statistically significant?P = 0.031?,there was no significant difference in AG diameter between the two groups?P = 0.221?.In the thrombus group,the straight sinus AG is the most common,followed by the superior sagittal sinus,right lateral sinus,and left lateral sinus,while in the control group,the superior sagittal sinus is the most common,followed by the left lateral sinus,right lateral sinus,and Straight sinus;CVST involves the largest number of AG detections at one site,followed by two sites,and AG is not detected at four sites.The AG detection rate is negatively correlated with the number of sites where CVST occurs.It can be seen that the more CVST sites,The lower the detection rate of AG,no matter what period of venous sinus thrombosis,the imaging performance of T2WI and Flair sequences is different from AG,which can realize the identification of CVST and AG.Conclusion The number of cases in which AG could be detected in the CVST group was less than that in the control group;the more thrombuses involved the venous sinuses,the detection rate of AG showed a decreasing trend;the T2WI and Flair sequences could effectively identify CVST and AG.
