Objective:By analyzing the clinical characteristics of 92 cases of idiopathic tinnitus,and using the anxiety self-rating scale and the tinnitus questionnaire score,the relationship between the clinical characteristics of tinnitus and the degree of anxiety and the severity of tinnitus was explored to provide basis for personalized diagnosis and treatment.Methods:92 idiopathic tinnitus patients had enrolled the outpatient Clinic and inpatient,ENT department of the affiliated hospital of Yan'an university,who were diagnosed from February 2018 to February 2019 in.We choose 92 patients with idiopathic tinnitus who were willing to cooperate as our study objects.The common data in 92idiopathic tinnitus patients were investigated:sex,age,occupation the part of tinnitus,the course etc.The audiology examination was conducted by unified professionals:Pure tone audiometry,acoustic conductance test and tinnitus matching test.After the tsxt,the patients filled in the questionnaire:Tinnitus Evaluation Questionnaire(TEQ),Self-rating Anxiety Scale(SAS).Results:1.In this study,Results male is higher than female,about 1.36:1;patients are mostly between 31 to 50 years old,the second is 21-30 years old and 51 to 60 years old,<and>20 patients with at least 70-year-old group,educational level,more concentrated in the patients with middle school,high school culture level,less patients with university or above the level of culture,professional distribution are mainly farmers and workers.2.In this study,the part of tinnitus:the unilateral tinnitus was the most common type,with 36 cases(39.1%)of left tinnitus,29 cases(31.5%)of right tinnitus,23cases(25.0%)of bilateral tinnitus,and only 4 cases(4.4%)of cranial tinnitus.The part of tinnitus was independent with sex(P>0.05).The pitch of tinnitus was mainly monotonic tinnitus about 57 cases(62.0%).There were 58 cases of buzzing(63.0%),12 cases of squeaking(13.0%)and 6 cases of running water(6.5%).The tone of tinnitus was high-frequency in 51 cases(55.4%),followed by 25 cases(27.2%)of low-frequency and 16 cases(17.4%)of medium-frequency.The loudness of tinnitus mainly was third-level included 53(57.6%)patients,followed by 20 patients(21.7%)with fourth-level,and the least patients with first-level and sixth-level were 1(1.1%)and 3(1)3.3%),the second-level in 8 cases(8.7%).33(35.9%)had normal hearing loss,35(38.0%)had mild hearing loss,21(22.8%)had moderate hearing loss,and 3(22.8%)had severe hearing loss.The number of patients with severe hearing loss was0;the difference between the frequency of tinnitus tone and the degree of hearing loss was not statistically significant(P>0.05);the difference between the level of conscious loudness and the degree of hearing loss was not statistically significant.(P>0.05);there is no correlation between the tone of the tinnitus and the degree of loudness and the degree of hearing loss.3.Of the 92 patients with idiopathic tinnitus in this study,the maximum number of patients with tinnitus severity II was 47(51.1%),followed by 22 patients with tinnitus level III(23.9%),and patients with tinnitus severity V.At least,there were 5cases(5.4%),the number of tinnitus severity I and IV were the same in 9 cases(9.8%);26 of them(28.3%)were associated with anxiety,mild anxiety(22.9%),and moderate anxiety(4%)(4.4%);the distribution of age segmentation in the severity of tinnitus was not statistically significant(2=6.56,P>0.05).There was no significant difference in the relationship between gender and tinnitus severity(Z=-0.745,P>0.05);there was no significant difference in the distribution of occupational tinnitus severity(?~2=7.074,P>0.05);There was no significant difference between the education level and the severity of tinnitus(?~2=4.512,P>0.05).The distribution of tinnitus in the severity of tinnitus was not statistically significant(?~2=3.053,P>0.05);tinnitus There was no significant difference in the distribution of the severity of the disease(?~2=1.629,P>0.05);the frequency of tinnitus was correlated with the severity of tinnitus,and it was correlated(r_s=0.242,P<0.05).);conscious loudness is related to the severity of tinnitus,becoming significant Degree correlation(r_s=0.754,P<0.01);the degree of hearing loss was correlated with the severity of tinnitus,and it was significantly correlated(r_s=0.358,P<0.01);the degree of anxiety associated with severity of tinnitus was correlated.Sexuality was significantly associated with moderate(r_s=0.520,P<0.01);meaning that the severity of tinnitus was not related to age,gender,education level,tinnitus duration,tinnitus,etc.;tinnitus tone frequency,tinnitus loudness,accompanied The degree of hearing loss,the degree of anxiety and other factors have a certain correlation with the severity of tinnitus anxiety,that is,the higher the frequency of tinnitus,the more severe the tinnitus;the higher the vocal ringing,the more severe the tinnitus;the greater the hearing loss,the more serious the tinnitus The higher the degree of anxiety associated with it,the more severe the tinnitus.Conclusion:1.Clinical features of tinnitus:In the idiopathic tinnitus patients in this study,males were more than female,the mainly patients were young person,and the education level was mostly in the junior high school and high school culture level.The occupation was mainly workers and farmers;the acute tinnitus accounted about half of the total number;the unilateral tinnitus was common type.In bilateral snoring and cranial snoring,the left tingled was the most common type in unilateral tinnitus,and the cranial sound is the least common type;the pitch of tinnitus was high-frequency of monotonic,and the loudness of tinnitus was mostly distributed in grades 3 and 4;more than half of the patients had different degree of hearing loss;the pitch and loudness of tinnitus are not related to the stage of hearing loss.2.Factors affecting the severity of tinnitus:The severity grade was most in level II of the tinnitus patients and the severity of tinnitus had no correlation between age,sex,occupation,education level,location,course of tinnitus and the severity of tinnitus.The pitch,loudness of tinnitus and with hearing loss,anxiety were correlated with the severity of tinnitus,and all of them have significant positive correlation with the severity of tinnitus.Therefore,we should pay attention to the psychological state of tinnitus patients and make individualized treatment plans according to our own conditions.The treatment of tinnitus requires the joint efforts of otolaryngologists,psychologists and psychiatrists.
