Objective:To compare the clinical efficacy and safety of anatomic hepatectomy and nonanatomic hepatectomy for hepatocellular carcinoma,and to provide a better treatment for patients with hepatocellular carcinoma.Methods:The clinical data of 55 patients with single hepatocellular carcinoma treated by hepatobiliary surgery from January 2012 to December 2016 in Hainan Medical College First Affiliated Hospital were divided into anatomical hepatectomy group(n=28)and non-anatomical hepatectomy group(n=27),two groups of patients before surgery(age,tumor size,positive rate of hepatit B,Child-Pugh classification,AFP,ALT,TBIL,ALB),intraoperative effect(operation time,blood loss,intraoperative blood transfusion rate,margin efficiency),postoperativeeffect(ALT,TBIL,CHE,ALB of postoperative 3 days,ALT,TBIL,CHE,ALB of postoperative 7 days,and the incidence of postoperative complications,perioperative mortality,diseasefreesurvival months,overall survival months)were compared.Results:There was no significant difference in the age,tumor size,positive rate of HBV,Child-Pugh classification,AFP,ALT,TBIL and ALB between the two groups(P=0.160,P=0.602,P=0.362,P=0.979,P=0.925,P=0.184,P=0.339,P=0.301).There was no significant difference between the two groups in intraoperative bloodloss,intraoperative blood transfusion rate and effective incision margin(P=0.375,P=0.072,P=0.206).However,the operation time of the anatomichepatictomy group was shorter and the difference was statistically significant(P=0.020).The ALT,TBIL,CHE,ALB of postoperative 3 days,ALT,TBIL,ALB of postoperative 7 days,1 year after the tumorfree survival rate,the overallpostoperative survival was no significant difference(P=0.156,P=0.572,P=0.289,P=0.965,P=0.971,P=0.339,P=0.338,P=0.384,P=0.427).CHE of postoperative 7days,the incidence of postoperative complications,postoperative tumorfree survival time of anatomical resection of hepatocellular carcinoma group hasobvious advantages(P=0.023,P=0.008,P=0.026).Conslusion:Anatomical resection of hepatocellular carcinoma has the advantages of shorter operation time,less postoperative complications,faster recovery of liver function,longer diseasefree survival time,obvious clinical effect and definite curative effect,and is worthy of promotion.
