Objective:CTA was used to construct the uterine fibroids arterial network,and the blood supply of fibroids was graded.Different treatment methods were selected according to blood supply classification,and the therapeutic effect was evaluated.The guiding significance of CTA on the choice of treatment methods for uterine fibroids was explored.Method:1.Study object selection: 50 patients with uterine fibroids who underwent pelvic CTA examination in our hospital from October to December 2017 were selected as the observation group.The uterine fibroids without pelvic CTA were selected in our hospital.Eighty-five patients were selected as control group,45 of whom underwent laparoscopic myomectomy,40 of whom underwent HIFU.2.Construction of fibroids arterial network and blood supply classification: 50 cases of observation group uterine fibroids arterial network were constructed by CT post-processing software.Quantitative density of vascular volume of uterus and fibroids was used to quantify the ratio of vascular volume of fibroids.The blood supply of uterine fibroids is divided into four types: blood flow type,blood flow type,general blood flow type,and less blood flow type.3.Determining the grouping and observation indicators: According to the blood supply type,the treatment of fibroids was selected,and the blood flow type and blood-rich flow type were performed by laparoscopic hysteromyoma removal,as observation group1;General blood flow type and less blood flow type was performed HIFU treatment after location by the HIFU center of our hospital,as observation group 2;laparoscopic myomectomy without CTA was control group 1;HIFU without CTA was control group 2;The average amount of bleeding during uterine myomectomy and the average ablation rate of uterine fibroids during HIFU ablation were used as indicators.4.Analysis of the results: statistical analysis of the observation indicators of the observation group and the control group,whether the difference is statistically significant,whether the CTA has a guiding significance for the choice of treatment methods for uterine fibroids.Results:1.Based on CTA,50 cases of uterine fibroids arterial network were successfully constructed in the observation group,including 8 cases of blood flow type,11 cases of rich blood flow type,26 cases of general blood flow type and 5 cases of less blood flow type.2.A total of 19 patients with abundant blood flow type and rich blood flow type were selected for laparoscopic myomectomy,which was observed group 1;A total of 31 cases of blood flow type and less blood flow type were located by the HIFU Center of our hospital,six of them were poorly safe ultrasound channel,and were not suitable for HIFU treatment,they were not enrolled,and the remaining 25 cases were suitable for HIFUtreatment,as observation group 2.3.The mean bleeding volume of laparoscopic myomectomy in observation group 1and control group 1 was(64.47 ± 21.08)ml and(127.33 ± 29.26)ml,respectively.Statistical analysis showed that P<0.05,statistically significant;observation group 2 and control group 2 HIFU-treated uterine fibroids had an average ablation rate of(86.80 ±5.38)% and(76.46 ± 4.16)%,respectively,and statistical analysis showed that P<0.05,with statistical difference;4.According to the analysis results,laparoscopic myomectomy and HIFU treatment of uterine fibroids according to CTA blood supply classification have good curative effect and have guiding significance for the choice of treatment methods for uterine fibroids.Conclusion:Based on CTA,the uterine fibroids arterial vascular network is constructed.Before the treatment,the blood supply of fibroids is clearly defined,and the choice of treatment methods has certain guiding significance,which is helpful for the individualized selection of treatment methods for uterine fibroids.
