| Objective Through the fetal brain MRI image,observe whether the corpus callosum is intact and whether there is complex corpus callosum dysplasia,and calculate the proportion.By measuring the size of the vermis of cerebellum,we compared the difference between the agenesis of corpus callosum fetus with the normal fetus,and provided reliable clinical evidence for prenatal counseling and related clinical intervention.With a view to reducing the birth rate of children with developmental defects in the nervous system.Metheds From January 2013 to December 2018,41 cases of middle and late pregnancy with agenesis of corpus callosum were diagnosed by prenatal ultrasound and fetal MRI in our hospital.The artifact of MRI images was small,and the pregnancy outcome of all parturients was clear.Choose 226 cases who performed prenatal ultrasound screening and fetal craniocerebral MRI at the same time and diagnosed as normal or mild lateral ventricle widening,and were followed up as normal or without obvious abnormalities after delivery.The median sagittal position was used to measure the area of vermis,and compare ACC fetuls’ s vermis with normal fetuls’ in order to observe whether there is difference.All data were processed by SPSS 22.0 statistical software.Results1.Among 41 cases,35 cases were complete agenesis of corpus callosum,5 cases were partial agenesis of corpus callosum and 1 case was hypoplasia corpus callosum.There were27 cases diagnosed as agenesis of corpus callosum and 14 cases diagnosed as complex agenesis of corpus callosum.2.In this study,40 cases(38 cases of induced labor,2 cases of delivery and 1 case lostfollow up)had definite pregnancy outcomes,including 14 cases of complex agenesis of corpus callosum,13 cases of induced labor,and 1 case of hypoplasia corpus callosum with HIE lost follow-up.The induction rate was 92.6%(25/27)in 28 cases of agenesis of corpus callosum.Two cases of childbirth were followed up by postpartum MRI.The follow-up results of 2 cases were consistent with prenatal MRI diagnosis:were agenesis of corpus callosum.3.Among 27 cases of agenesis of corpus callosum,,5 cases had sulcus retardation,4cases had hemispheric fissure cyst and 4 cases had arachnoid cyst,14 cases diagnosed as complex agenesis of corpus callosum,1 case had chromosomal abnormality(trisomy 18),others including midbrain fissure malformation,parietal occipital lobe dysplasia,longitudinal fissure hematoma,meningocele,multiple microgyrus malformation,once each occurrence,porencephalia occurred 3 times,Dandy-Walker malformation occurred 4 times,hydrocephalus occurred 6 times,and 1 case of hypoplasia corpus callosum combined with HIE.4.Without 4cases diagnosed as Dandy-walker malformation,the rest 37 cases measured cerebellar vermis area.Among them,the cerebellar vermis area in 18 cases was smaller than that of normal fetuses at the same gestational week.Conclusion1.In agenesis of corpus callosum,complete agenesis of corpus callosum is the most common type,followed by partial agenesis of corpus callosum,hypoplasia corpus callosum is less common,and complex agenesis of corpus callosum is less common than agenesis of corpus callosum.2.When there is corpus callosum dysplasia,MRI can well show intracranial concomitant malformation:agenesis of corpus callosum can be accompanied with hemispheric fissure cyst and sulcus gyrus retardation.Complex agenesis of corpus callosum is often accompanied with severe brain structural abnormalities such as porencephalia,Dandy-walker malformation and hydrocephalus.3.The median sagittal position of MRI can better evaluate the development of the corpus callosum.And when the corpus callosum is incomplete,by measuring the area of cerebellar vermis at the median sagittal position can evaluate the development of cerebellar vermis. |
