Comparative Analysis Of One-stage Suture And T-tube Drainage In Laparoscopic Common Bile Duct Exploration

Objective:To compare the efficacy of laparoscopic common bile duct exploration with first-stage suture(LCBDE+PDC)and laparoscopic common bile duct exploration with T-tube drainage(LCBDE+TTD)in the treatment of common bile duct stones,to investigate and compare the incidence of short-term and long-term complications after surgery,and Comparison of operation time,postoperative hospital stay,hospitalization expenses,etc.Methods: This study was a retrospective clinical data analysis of patients who underwent LCBDE+PDC and LCBDE+TTD treatment for common bile duct stones in the Wuhan Central Hospital of Jianghan University from June 2013 to November 2018.Data were followed up by out patient-clinic or by reviewing follow-up hospitalization data.Results:A total of 100 patients were enrolled in the study,including 50 in the LCBDE+PDC group and 50 in the LCBDE+TTD group.There was no significant difference in the treatment of choledocholithiasis between the two groups.Patients in the LCBDE+PDC group had significantly fewer hospital stays than the LCBDE+TTD group,and the probability of postoperative biliary infection in the LCBDE+PDC group was significantly lower than in the T-tube drainage group.There was no significant difference on other early complications,long-term complications,hospitalization time,hospitalization expenses,etc.Conclusion: The first-stage suture did not increase the risk of postoperative bile leakage,and reduced the probability of biliary infection,avoiding the complications associated with T-tube;hospitalization costs,operation time was not extended compared with T-tube drainage,and postoperative hospitalization time was obvious Shortened;therefore,under the premise of strict control of indications,laparoscopic common bile duct exploration is feasible,safe and cost-effective.
