Research backgroundAccording to estimates by the World Health Organization,by 2025,the number of people with diabetes will reach 300 million.There is no cure for the lifestyle disease of diabetes,and health education has become the basic work to control diabetes.Nurses have become the main force in undertaking health education.At present,the lack of scientific,systematic and standardized health education model for diabetic patients in China,the lag of clinical health education,the lack of health knowledge,poor understanding and living habits of diabetic patients seriously affect their health level and well-being.Research objective1.To understand the status of diabetes knowledge,self-management ability and subjective well-being of type 2 diabetes patients;2.To evaluate the intervention effect of LEARNS health education model on knowledge and beliefs and subjective well-being of patients with type 2 diabetes,to understand the learning needs of patients with type 2 diabetes,and to provide new strategies for health education for patients with type 2 diabetes.MethodsThis study uses a class-like experimental design consisting of two phases.Phase 1:Baseline assessment and analysis.A convenient sampling method was used to select 320 hospitalized type 2 diabetic patients with two(two groups)endocrine wards in Lanzhou who met the inclusion criteria.The Diabetics Knowledge Test(DKT)was used to manage diabetes self-efficacy.Diabetes Self-Efficacy Scale(DSMES),Newfoundland Memorial University Happiness Scale(MUNSH),with mean ± standard deviation,percentage description,multiple linear analysis as a statistical method to understand diabetes knowledge,beliefs,subjective well-being status.Phase 2: intervention and effectevaluation.The subjects were divided into the control group and the experimental group according to the admission wards.The experimental group gave the LEARNS health education model,and used the diabetic sodalities learning needs assessment form(self-made)to conduct a study of the needs of type 2 diabetes patients,and the control group gave regular health education.Three months after the intervention,the t-test was used to compare diabetes knowledge,belief,and subjective well-being between the two groups.ResultsCurrent status survey results:(1)The diabetes cognitive questionnaire scored 10points-100 points,with an average of 70.93 ± 19.08 points.The educational level,course of disease and body mass index were the main factors affecting the score of the knowledge questionnaire of the research subjects.(2)The self-efficacy level of patients with type 2 diabetes was not ideal,with a score of 6 to 100 points,with an average of71.40±21.22 points.The lower scores were: blood glucose monitoring,regular exercise,and diet control.The main factors affecting the self-administration efficacy scores of the subjects were gender and body mass index.(3)The subjective happiness measure score of diabetic patients was 8 points-32 points,with an average of 20.64±5.09 points,which was at the middle and low level.The main factors affecting the subjects' subjective happiness scores were age,education level and disease duration.Intervention results:(1)There was no difference in general data and subjective happiness of diabetes between the two groups(P>0.05).(2)Diabetes knowledge questionnaire score(76.51±18.52)and diabetes self-administration efficacy score(82.17±19.80)and subjective happiness score(22.14±5.12)increased after the intervention group(P<0.05);The score of knowledge questionnaire(86.30±14.11)was improved(P<0.05),but the self-management efficacy of diabetes(68.90±19.82)and subjective happiness(20.16±5.04)were not improved(P>0.05).Conclusion1.Inpatients with type 2 diabetes lack knowledge about diabetes,lack of self-management efficacy,and have low subjective well-being.2.Using the LEARNS model can enhance the patient's knowledge of diabetes,Especially for the improvement of the ability to manage their own health and thesubjective well-being of diabetic patients.
