The Effects Of Different Health Education Programs On Type 2 Diabetes Patients

Background The prevalence of Type 2 diabetes is at high epidemic level in the world, while the diabetes health education is considered as an important part of clinical diabetes management.However,the most effective method for delivering education and teaching self-management skills is under established.Objective To compare the effect of different diabetes health education programs on the level of diabetes knowledge,self-management skills,metabolic control and medical expenditure in patients with type 2 diabetes mellitus.Methods 80 patients with type 2 diabetes mellitus were enrolled and patients were assigned to intensive diabetes health education program(treatment group,n=40) or conventional program(control group,n=40).The treatment group received diabetes self-management education for 5 sequential days followed by subsequent regular advice. In the control group,subjects received education once a week for up to five weeks(whole day each time) followed by regular advice.A Chinese version of the Diabetes Knowledge Scale and Diabetes self-management Scale were used to assess the level of diabetes knowledge and self-management skills.The changes of metabolic control and medical expenditure were assessed after 3 and 6 months.Results(2) Scores of diabetes knowledge and self-management skills were significantly increased in each group at 3 and 6 month.HbAlc in intensive group was decreased by 2%on average,while 1.2%in control.The percent of patients who reached HbAlc<6.5%in intensive group was increased from 27.50%to 80.00%(3 month) and 72.50%(6 month)(P<0.05,respectively).The percent of patients reached HbA1c<6.5% in control was increased from 27.50%to 38.46%(3 month),43.59%(6 month)(P>0.05, respectively).The percent of patient with BMI<24 increased from 40.0%to 65.5%(3 month),67.5%(6 month)(P<0.05,respectively),while no similar change was found in control group.Medical cost in 4-6 month was less than that of 1-3 month in both groups. (2) Compared with control group,scores of diabetes knowledge,self-management skills, compliance and the percent of patients HbA1c<6.5%were significantly increased in intensive group(P<0.05);The health-care cost was significantly less in intensive group than control(P<0.05)while the cost for self-monitoring of blood glucose was significantly higher in intensive group than in conventional one during 4-6 month of follow-up(P<0.05).Conclusion Intensive education program was more effective than conventional program on improvement of diabetes knowledge,self-management skills,HbAlc and BMI.Whole cost of health care during program is no changed while the drug tee decreased and lab fee increased.
