Objective: To analyze the risk factors of the severity of enlarged perivascular spaces(EPVS)in elderly patients.Methods: A total of 264 elderly patients with EPVS detected by craniocerebral MRI in the First Affiliated Hospital of China Medical University from January 2017 to January 2019 were selected as subjects of this study.According to the location,number and size of EPVS on brain MRI,the patients were divided into four groups: group A(EPVS = 1),group B(EPVS = 2),group C(EPVS = 3),and group D(EPVS = 4).General clinical data and blood biochemical indicators were collected in detail,and the number of old lacunar infarction(LI)was recorded.The relationship between the severity of EPVS and NSE,the number of old LI and other risk factors was analyzed by univariate analysis and multivariate ordered logistic regression analysis.The correlation between EPVS and risk factors was analyzed by Spearman correlation analysis.Result: A total of 264 elderly patients with EPVS were included in the study.After counting and grading EPVS in basal ganglia and central semioval area,EPVS was divided into four groups according to the severity of EPVS.Univariate analysis of basal ganglia revealed that there were differences among the proportion of hypertension,neuron-specific enolase(NSE),age,and number of old LI among the four groups,some of which also showed statistical significance(P < 0.05);Taking the significant factors mentioned above as independent variables and the severity of EPVS in basal ganglia as dependent variables,the results of multivariate logistic regression analysis showed that age,NSE,the number of old LI and hypertension were independent risk factors for the severity of EPVS in basal ganglia of elderly patients.There was no statistical difference among the four groups in the centrum semiovale(P < 0.05);Spearman correlation analysis showed that the severity of EPVS in the basal ganglia was positively correlated with age,hypertension,coronary heart disease,NSEand the number of old LI,and the severity of EPVS in the centrum semiovale was positively correlated with the number of old LI.Conclusion: There were significant differences among risk factors of EPVS in different parts and degrees of elderly patients.Age,NSE,the number of old LI and hypertension were independent risk factors for EPVS severity in basal ganglia of elderly patients.They were positively correlated with age,hypertension,coronary heart disease,NSEand the number of old LI,while there was no significant difference among risk factors of EPVS in the centrum semiovale.Only the severity of EPVS in the centrum semiovale was positively correlated with the number of old LI.
