| Objective: To explore the value of serum IgG4,serum CA 19-9 and blood routine indexes in the clinical differential diagnosis of autoimmune pancreatitis and pancreatic cancer Method: From 2012 to 2018,49 patients with autoimmune pancreatitis(AIP)and 49 patients with pancreatic cancer(PC)diagnosed in shengjing hospital were selected as the study subjects,and the differences in serum IgG4,serum ca19-9,blood routine indexes,total bilirubin,lipase,amylase and other indexes between the two groups were statistically compared.Univariate Logistic regression analysis was performed on the indicators with statistical significance.To explore the correlation analysis of serum IgG4 with serum ca19-9,eosinophil count(EO)and hemoglobin,respectively draw the ROC curve of serum IgG4,eosinophil count(EO),serum ca19-9 and their combined diagnosis in the differential diagnosis of AIP group and PC group,and calculate the area under the ROC curve(AUC).Results:1.Serum ca19-9 and hemoglobin in the AIP group were lower than those in the PC group,and eosinophil count(EO)and serum IgG4 were higher than those in the PC group,with statistically significant differences(P<0.05).2.Serum ca19-9 was negatively correlated with serum IgG4 in the two groups(r=-0.401,P<0.05),eosinophil count(EO)was positively correlated with serum IgG4(r=0.307,P<0.05),and hemoglobin was negatively correlated with serum IgG4(r=-0.501,P<0.05).3.In the differential diagnosis of AIP group and PC group,the diagnostic boundary value of serum IgG4 was 2.11g/L,the sensitivity was 0.857,and the specificity was1.The diagnostic threshold of serum ca19-9 was 74.30U/ml,with a sensitivity of 0.857 and a specificity of 0.714.The diagnostic threshold of eosinophil count(EO)was0.18*109/L,with a sensitivity of 0.633 and a specificity of 0.796.Conclusion:1.Serum IgG4 is still the most valuable in the diagnosis of autoimmune pancreatitis serological index,serum CA 19-9,eosinophil count in autoimmune pancreatitis(AIP)and pancreatic cancer(PC)in the differential diagnosis significance,will recommend IgG4 levels increase to 2.11 g/L,reduce the patients with autoimmune pancreatitis(AIP)the risk of needless surgery. |
