Objective: Periodontitis(PD)is a chronic infectious disease caused by pathogenic microorganisms in dental plaque.The prevalence of periodontitis in adults in China is 40% to 60%,which is the leading cause of loss of teeth in adults.Rheumatoid arthritis(RA)is a systemic disease characterized by chronic symmetrical polyarthritis.Periodontitis has similar clinical manifestations as rheumatoid arthritis,which are alveolar bone loss,synovitis and joint damage caused by leukocyte infiltration and inflammatory mediators,respectively.In this paper,we examined the periodontal clinical indexes of patients with CP and RA and matrix metalloproteinase-3(MMP-3)and matrix metalloproteinase-9(MMP-9)in serum and gingival crevicular fluid.The relationship between the expression of MMP-3 and MMP-9 and CP and RA were investigated.Methods: According to the inclusion criteria,24 patients with CP and RA who met the conditions of rheumatology and immunology at Shengjing Hospital affiliated to China Medical University from March 2017 to July 2018 [CP+RA group,age(56.58±9.04)years old,4 males and 20 females] and 22 cases of RA patients [RA group,age(49.63±11.69)years old,5 males and 17 females];18 patients with simple CP in the Department of Stomatology,Shengjing Hospital affiliated to China Medical University [CP group,age(51.44±12.31)years old,7 males and 11 females];16 healthy controls of Shengjing Hospital Medical Center affiliated to China Medical University [healthy control group,age(50.88±9.52)years old,6 males and 10 females];There were no significant differences in age and gender between the 4 groups.The general condition,depth of probing,clinical attachment loss(CAL),sulcus bleeding index(SBI),simplified calculus index(CI-S),simplified soft scale index(DI-S);serum and gingival crevicular fluid samples were collected from each group,and the expression levels of MMP-3 and MMP-9 in serum and gingival crevicular fluid were detected by enzyme-linked immunosorbent assay.Correlation analysis between MMP-3 and MMP-9 expression levels and periodontal index in 4 groups of subjects.Results: The indexes of PD,CAL,SBI,CI-S and DI-S in CP+RA group were significantly higher than those in group H and RA.The indexes of PD,SBI,CI-S and DI-S in RA group were significantly higher than those in group H;The PD,CAL,SBI,CI-S,and DI-S indicators in CP group were significantly higher than the RA and H groups.The expression level of MMP-9 in serum was significantly higher in CP+RA group [(2671.71±1258.33)?g/L] than in RA group,CP group and H group [1395.29±750.16),(511.38±539.52)?(67.13±34.95)?g/L];The expression level of MMP-9 in gingival crevicular fluid was significantly higher in CP+RA group and RA group [(26.11±5.25)?(24.35±4.70)?g/L] than in H group and CP group [1.04± 0.58),(18.83±11.87)?g/L];The expression level of MMP-3 in serum was significantly higher in CP+RA group and CP group [11606.31±7195.73,(9469.48±7152.98)?g/L] than in RA group and H group [(2389.85±1320.30),(1502.96±309.31)?g/L];The expression level of MMP-3 in gingival crevicular fluid was significantly higher in CP+RA group [(165.82±45.59)?g/L] than in RA group,CP group and H group [48.97±18.07)?(71.99±88.23)?(18.69±7.25)?g / L]?The CAL index of CP+RA group was positively correlated with the expression level of MMP-9 in serum(r=0.69,P=0.00).The SBI index of CP+RA group was positively correlated with the expression level of MMP-9 in serum(r=0.47,P=0.02).The CI-S index of CP group was positively correlated with the expression level of MMP-9 in serum(r=0.50,P=0.03).The DI-S index of CP group was positively correlated with the expression level of MMP-9 in serum(r=0.71,P=0.00).The PD value of CP+RA group was positively correlated with the expression level of MMP-3 in serum(r=0.49,P=0.02);the PD value of RA group was positively correlated with the expression level of MMP-3 in serum(r=0.45,P=0.04);the CI-S index of CP group was positively correlated with the expression level of MMP-3 in serum(r=0.51,P=0.01);the CAL index of CP+RA group was positively correlated with the expression level of MMP-3 in gingival crevicular fluid(r=0.57,P=0.00);The expression of MMP-3 in gingival crevicular fluid was positively correlated with CAL in CP group(r=0.49,P=0.04)Conclusions: The levels of MMP-3 and MMP-9 in serum and gingival crevicular fluid of patients with RA and CP were significantly increased.MMP-3 and MMP-9 may be associated with the development of CP and RA.
