Objective:By detecting the expression of matrix metalloproteinase-13(MMP-13)and tissue inhibitor of metalloproteinase-1(TIMP-1)in gingival crevicular fluid of patients in Chronic Periodontitis(CP)with Rheumatoid arthritis(RA),to investigate the expression of MMP-13 and TIMP-1 in CP with RA,and analyze the correlation between the expression level of the two factors in gingival crevicular fluid and periodontal indexes and rheumatic immune indexes,so as to further understand the correlation between the two diseases at the molecular biology level,which is helpful for clinicians to provide theoretical basis for the development of personalized treatment plan for patients with CP with RA.Method:1.From Novermber 2018 to Novermber 2019,22 patients with CP and RA[CP+RA group,(52.86±12.7)years old,15 females and 7 males],20 patients with RA only [RA group,(55.9±14.49)years old,15 females and 5 males] in the Department of Rheumatology and Immunology of Liaoning Provincial People's Hospital,24 patients with simple CP in the Department of Stomatology,Liaoning Provincial People's Hospital [CP group,(54.62±10.73)years old,9 females and 15 males] and 18 generallyhealthy people [H group,(46.89±8.41)years old,12 females and 6 males] were recruited.There were no significant differences in age and gender among the 4 groups.2.(1)General data was collected in patient's age,gender,course of RA,smoking history,BMI,oral hygiene habits,etc.(2)All subjects' epidemiological indices of tooth position(11?16?17?26?27?31?36?37?46?47,There should be at least one exponential tooth in each quadrant,and if both are missing,the neighboring teeth should be measured.)received periodontal examinations of plaque index(PLI),gingival index(GI),bleeding on probing(BOP),probing depth(PD),clinical attachment loss(CAL)and missing teeth(MT).(3)The relative indexes of rheumatic immunity in RA group and CP + RA of erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),rheumatoid factor(RF),anticycliccitrullinated peptide antibody(ACCP)were from Liaoning Province People's Hospital of clinical laboratory.(4)The gingival crevicular fluid(GCF)were collected by moisture-absorbing paper tip weighing method,a tooth with the highest PD was used as the experimental molar tooth and the area with the deepest PD was the GCF sample site,and the samples were fozen and stored at-70?.(5)Enzyme-Linked Immunosorbent Assay(ELISA): the levels of MMP-13 and TIMP-1 in gingival crevicular fluid were determined by a double antibody sandwich method.(6)The spss 20.0 software package was used to analyze the data.Result:1.Comparison of general data:there had no significant differences on age,gender,and smoking in each group(P>0.05).CP+RA group and RA group had no significant differences on course of the disease and BMI(P>0.05).2.The 4 groups had statistically significant differences on brushing condition and use of floss or interdental brushes(P<0.05),but no significant difference in the rate of regular oral examination(P>0.05).3.When comparing the clinical indicators of severe periodontitis,the indices of PLI,GI,BOP,PD,CAL,MT in the CP+RA group and CP group were significantly higher than those in the RA group and H group(P<0.05),the indices of PLI,PD,CAL in the CP+RA group were significantly different from those in the CP group(P<0.05),the indices of GI,BOP in the CP group were significantly higher than those in the CP+RA group(P<0.05);the indices of PLI,PD in the RA group were significantly higher than those in the H group(P<0.05).4.The expression of MMP-13,TIMP-1 in gingival crevicular fluid of CP+RA group were significantly higher than that of CP group and RA group(P<0.05).The expression of MMP-13,TIMP-1 in gingival crevicular fluid of CP+RA group and CP group and RA group were significantly higher than that of H group(P<0.05).5.The expression level of MMP-13 and TIMP-1 in gingival crevicular fluid was positively correlated with PLI,GI,CAL and PD.6.The expression level of MMP-13 was positively correlated with ESR,CRP and ACCP expression of in both groups.The expression level of TIMP-1 in gingival crevicular fluid was positively correlated with ESR and CRP expression of in both groups.7.The expression levels of CRP,ESR,RF and ACCP in CP+RA group were significantly higher than those in RA group,with statistically significant differences(P< 0.05).Conclusion:1.The levels of MMP-13 and TIMP-1 in gingival crevicular fluid of CP patients with RA were significantly increased.MMP-13 and TIMP-1 may be correlated with the occurrence and development of CP and RA.2.RA may have an aggravating effect on CP.CP may promote RA to some extent.
