Objective: To investigate the expression characteristics of angiopoietin-like protein4(ANGPTL4)in serum and urine of patients with hyperlipidemia related proteinuria and its correlation with blood lipids and urine protein.Methods: A total of 182 patients who were hospitalized in Gansu Provincial Peo ple's Hospital from January 2017 to December 2018 were enrolled: 68 patients with h yperlipidemia related proteinuria group(HL-Pro group);68 patients of hyperlipidemia with non-proteinuria group(HL-NPro group)and 46 patients with non-hyperlipidemia r elated proteinuria group(NHL-Pro group).The healthy control group(Con group)was from 50 healthy subjects in the physical examination center in Gansu Provincial Peop le's Hospital.The expression levels of ANGPTL4 in serum and urine of four groups were quantitatively detected by enzyme-linked immunosorbent assay.The concentrations of total cholesterol(TG)?triglycerides(TC)?high-density lipoprotein(HDL)?low-den sity lipoprotein(LDL)?serum creatinine(Scr)?blood urea nitrogen(BUN)and 24-hour urine protein(24hUPro)were detected by automatic biochemical analyzer.Statistical a nalysis was performed on the data using SPSS 20.0 statistical software.Results: 1.ANGPTL4 levels of serum in HL-Pro group [27.94(18.90,53.72)] n g/ml were significantly different from those in HL-NPro group(53.32±24.01)ng/ml an d the NHL-Pro group(23.56±9.30)ng/ml.(P<0.05).They are not difference compared with Con group(36.82±17.03)ng/ml(P>0.05).2.Urine ANGPTL4 levels in the HL-Pro group [52.01(45.25,79.79)] ng/ml were significantly different from the HL-NPro group [9.96(8.35,12.43)] ng/ml,Con group(10.08 ± 2.38)ng/ml.Significance(P<0.05).They are not difference compared with N HL-Pro group(69.41±55.36)ng/ml(P>0.05).3.There are not correlation between serum and urine ANGPTL4 in the four grou ps(P>0.05).4.Serum ANGPTL4 levels in patients with hyperlipidemia(HL-Pro/HL-NPro grou p)were positively correlated with TC and TG.The levels of serum ANGPTL4 in diff erent TC and TG levels were different(P<0.05).However,there was not correlation b etween urine ANGPTL4 levels and TC?TG levels(P>0.05).5.Urine ANGPTL4 was positively correlated with 24 hUPro in patients with protei nuria(HL-Pro/NHL-Pro group).There were differences in urine ANGPTL4 levels betw een different 24 h proteinuria levels(P<0.05),but there was not correlation between Se rum ANGPTL4 and 24 hUPro.Conclusion: 1.ANGPTL4 of serum was significantly positively correlated with bl ood lipid levels and was not associated with kidney damage.It needs for further stud y that the value of ANGPTL4 in the development of hyperlipidemia related proteinuri a.2.ANGPTL4 of urine is positively correlated with the degree of proteinuria and has nothing to do with blood lipid levels.It may play an important role in the mecha nism of patients with hyperlipidemia related proteinuria.
