The Clinical Application Of Single-site Single-channel Retroperitoneal Laparoendoscopic Adrenalectomy

[Objective] To evaluate the safety and clinical efficacy of single-site single-channel laparoscopic technique in the treatment of adrenalectomy.[Methods] From January 2016 to October 2018,74 patients performed retroperitoneal laparoscopic adrenalectomy for adrenal tumors were randomly divided into experimental and control group.35 cases of experimental group were performed single-site single-channel retroperitoneal laparoendoscopic adrenalectomy with 16 males and 19 females,aged 43.14(+10.68),body mass index(BMI)23.75(+3.07)and tumor diameter 3.00(+0.69cm).In the control group,39 patients underwent routine retroperitoneal laparoscopic adrenalectomy,17 males and 22 females,aged 43.46(+11.53),body mass index(BMI)24.23(+3.47)and tumor diameter 2.77(+0.69cm).There were no significant differences in age,gender,BMI,tumor side,and maximum tumor diameter between the two groups(P>0.05).The operation time,intraoperative blood loss,incision length,incisionnumber,postoperative pain Verbal rating scale(VRS),drainage volume,postoperative hospital stay and other data for statistical analysis.[Results] The procedures of all patients were successful.In the experimental group,only one port was used in 6 cases,two ports in 23 cases,and three ports in 6 cases.39 cases in the control group were all performed with 3 ports.Of the two groups,the intraoperative blood loss(23.37±12.02 ml vs 37.72±15.86 ml,P<0.05),incision length(2.13±0.66 cm vs 3.51±0.65 cm,P<0.05),incision number(2.00±0.59 个 vs 3.00±0.00 个,P<0.05),postoperative pain Verbal rating scale(VRS)(1.63±0.69 vs 2.33±0.62,P<0.05),postoperative drainage(24.29±12.07 ml vs38.59±18.28 ml,P<0.05),postoperative hospital stay(4.69±0.83 d vs 5.18±1.07 d,P < 0.05)had singnificant difference.There was no significant difference in operation time and postoperative drainage time.[Conclusions]Single-site single-channel retroperitoneal laparoscopic adrenalectomy is safe and feasible for renal masses.compared with conventional retroperitoneal ones,it may be less trauma to the retropertoneal tissue for it’s reduction of dissecting retropertoneal tissue,and it was better on cosmetics.
