| Objective Through the study of risk factors,pathogen distribution and drug resistance of urinary tract infection in BPH patients in Xining area,it is hoped that the diagnosis and treatment of urinary tract infection in this area,especially for clinical use,has certain reference value.Methods A total of 945 patients with BPH,who were admitted to the Urinary Surgical Department of the Affiliated Hospital of Qinghai University from January 2017 to December 2018,were enrolled in the study.Collect and organize the patient’s age,physical condition,underlying disease,clinical manifestations,laboratory tests,urine bacterial culture and drug sensitivity,imaging examination,catheterization times,surgery,hospitalization time and other data for statistical analysis.Results 1,The independent risk factors for UTI in patients with BPH include age,diabetes,indwelling catheter,urodynamic examination,TURP,hospitalization time and so on.2,The main pathogen for UTI in patients with BPH is Gram-negative bacteria,of which Escherichia coli,Pseudomonas aeruginosa,Klebsiella pneumoniae are most common.The most common of Gram-positive bacteria are Staphylococcus epidermidis,Enterococcus faecium,Enterococcus faecalis.The proportion of fungi is small.3.The resistance rate of Escherichia coli to imipenem,amikacin,piperacillin/tazobactam was lower,and the resistance rates of antibiotics such as cephalosporins,quinolones and sulfonamides were higher.4.The resistance of ESBLs-producing Escherichia coli to other antibacterial drugs except for some β-lactamase inhibitor complexes,carbapenems and some aminoglycoside antibiotics was significantly higher than that of non-ESBLs-producing strains.Conclusion 1.The risk factors for UTI in patients with BPH include age,diabetes,urinary calculi,residual urine volume,indwelling catheterization,hospitalization time,urodynamic examination and TURP,and age,diabetes,indwelling catheter,urodynamic examination,TURP,hospitalization time are independent risk factors.2.The pathogen of UTI in patients with BPH in Xining area still uses Gram-negative bacilli as the primary pathogen,and the proportion of fungi and Staphylococcus aureus is small.3.The resistance rate of Enterococcus faecium to various antibiotics was significantly higher than that of Enterococcus faecalis.No enterococci strain resistant to vancomycin and linezolid was found.The proportion of ESBLs-producing strains in E.coli was higher than that of non-ESBLs-producing strains,and the resistance to E.coli produced by ESBLs was significantly higher than that of non-ESBLs-producing Escherichia coli.5.For urinary tract infections caused by Enterobacteriaceae producing ESBLs,comprehensive antibiotic susceptibility test results and clinical severity should be used to determine the antibiotic treatment plan. |
