The Morphology And Vascular Changes Of Retina And Choriocapillaris In Hla-b27-positive Anterior Uveitis

Objective: To observe the morphology and vascular changes of retina and choriocapillaris in patients with HLA-B27-positive anterior uveitis,and explore the relationship between vessel and the prognosis of anterior uveitis.Methods: Collecting acute anterior uveitis and age-matched normal subjects in Shanghai General Hospital between 2016.5 and 2017.1.Retinal thickness was measured by Optical Coherence Tomography(OCT).The density of superficial capillary plexus(SCP),deep capillary plexus(DCP)and choriocapillaris(CC)were measured by optical coherence tomography angiography(OCTA).We compared with the posterior segment of the effected eyes,the fellows and the normal ones in different groups.We also focused on the relationship between vessel and the prognosis of anterior uveitis.Results:(1)The thickness of RT-3mm in uveitis was much thicker than the fellow eyes' and the normal ones(P<0.05).The vascular density of SCP-3mm,CC-3mm in the effected eyes with acute anterior uveitis were significantly lower than the fellow eyes and the normal subjects(P<0.05).(2)The thickness of RT-3mm with HLA-B27-positive anterior uveitis was significantly thicker than the fellow eyes and the normal ones(P<0.05).The density of SCP-3mm and CC-3mm in the effected eyes were significantly lower than the fellows and the normal ones(P<0.05).(3)There were no significant differences among the thickness of RT-3mm in the effected eyes,the contralateral eyes and the normal ones in HLA-B27-negative patients(P ? 0.05).Neither were the density of SCP-3mm and DCP-3mm significantly different among them(P?0.05).(4)The density of SCP-3mm and DCP-3mm in HLA-B27-positive anterior uveitis were much lower than that in HLA-B27 negative uveitis(P<0.05).There was no significant difference in the thickness of RT-3mm,the density of SCP-3mm,DCP-3mm between HLA-B27 positive and negative patients(P ? 0.05).(5)With the improvement of inflammation in HLA-B27-positive anterior uveitis,the thickness of RT-3mm increased first and then decreased,while the density of retina and choriocapillaris showed a gradual upward trend.There was a time lag between the changes of vessel and the inflammatory state.Conclusion:(1)In anterior uveitis,the vascular density of retina and choriocapillaris decreased,while the retina thickness increased.(2)The density of retina and choriocapillaris in HLA-B27-positive anterior uveitis was much lower than HLA-B27 negative uveitis.(3)With the improvement of HLA-B27-positive anterior uveitis,the thickness of retina increased first and then decreased,the density of retina increased gradually.
