Objective:Systemic lupus erythematosus(SLE)is an autoimmune disease involving multiple systems and multiple organs.There are many women of childbearing age,and there are many autoantibodies in the body.The basic pathological changes are immune complex-mediated vasculitis.With the continuous research and improvement of SLE,the mortality rate of SLE has been greatly reduced.Cardiovascular disease(CVD)causes about 50% of SLE deaths.Among them,atherosclerosis has been considered as one of the important causes of SLE death.Dyslipidemia is a key factor in atherosclerosis.The reduction of high-density lipoprotein is an independent risk factor for cardiovascular disease.To further investigate the effect of immunological abnormalities on blood lipid metabolism during the onset of SLE,we have not treated the initial diagnosis.The blood lipids of SLE patients were tested and analyzed.Methods:144 cases of SLE initial treatment were included in the Department of Rheumatology and Immunology,the First Affiliated Hospital of China Medical University from February 2014 to 2017.There were 124 females and 20 males.Divided into dyslipidemia group and normal blood lipid group,choose complement,ESR,CRP,24-hour urine protein quantification,anti-dsDNA antibody as parameters,record the incidence of dyslipidemia in patients with SLE initial treatment;compare and analyze the changes and significance of clinical parameters in the two groups.Results:1)Among 144 patients,32 patients(22.2%)had normal lipids,112(77.8%)had dyslipidemia,and 54(48.2%)had elevated triglyceride(TG)in total dyslipidemia.(Total cholesterol,TC)increased 12 cases(10.7%),LDL cholesterol(Low density lipoprotein cholesterol,LDL-C)increased 12 cases(10.7%),high density lipoprotein cholesterol(high density lipoprotein cholesterol,HDL-C)was reduced in 95 cases(84.8%).Among them,the increase in TG and the decrease in HDL-C were most significant.2)CRP was significantly higher in the dyslipidemia group than in the normal blood lipid group(P<0.01),but there was no significant difference in the C3,C4,ESR,and 24-hour urine protein levels between the two groups(P>0.05).3)There was a statistically significant difference between the two groups in the incidence of elevated ESR and elevated CRP(P<0.05),but decreased in C3,C4 decreased,anti-dsDNA(+),24-hour urine protein quantitation >0.5g There was no statistical significance in the rate;4)There was no correlation between TG and inflammatory factors in the correlation analysis;TC was positively correlated with C4(r=0.179,P=0.032<0.05),and LDL-C was positively correlated with C4(r=0.188,P=0.024<0.05),TC,TG,LDL-C,HDL-C were positively correlated with 24-hour urine protein quantitation(P<0.05);TC was negatively correlated with ESR(r=-0.221,P=0.008<0.05).LDL-C was negatively correlated with ESR(r=-0.221,P=0.008<0.05);HDL-C was negatively correlated with ESR and CRP。 Conclusion:There was a significant dyslipidemia in patients with SLE,which was mainly due to elevated TG(48.2%)and decreased HDL-C(84.8%).In the group,the CRP level in patients with dyslipidemia was significantly higher than that in normal blood lipid group.The proportion of ESR and CRP increased in the dyslipidemia group was significantly higher than that in the normal blood lipid group.The HDL-C in SLE patients was correlated with ESR,CRP and 24-hour urine protein。...
