Objective:to study the correlation between glomerular filtration rate and carotid artery stenosis in patients with acute cerebral infarction.Method:220 patients with clinically diagnosed acute cerebral infarction were selected according to the results of head CT,MRI and other examinations. After admission, all the enrolled patients needed to improve renal function and other related laboratory examinations, including the estimation of glomerular filtration rate.According to the results of carotid artery ultrasound examination, the degree of great vessel stenosis was divided into:no or mild stenosis(0%~29%),moderate stenosis(30%~69%)and severe stenosis and occlusion(70%~100%).Results according to the GFR(glomerular filtration rate)is divided into three groups:high level group:eGFR≥80ml/(min/1.73*m~2),the middle level of groups:60ml/(min/1.73*m~2)≤eGFR≤79ml/(min/1.73*m~2),the low level groups:eGFR≤59ml/(min/1.73*m~2).Result:1)compared with the group without carotid artery stenosis, creatinine increased in the group with different degrees of stenosis, and eGFR decreased in the group with different degrees of stenosis, the difference was significant(P<0.01).2)in patients with acute cerebral infarction, there was a significant difference in the degree of carotid artery stenosis between the eGFR normal group and the mild decline group and the moderate decline group(P<0.05),and the level of eGFR was closely related to the degree of carotid artery stenosis(significance-double-tailed.000).3)Logistic regression analysis showed that eGFR may be an independent risk factor for carotid artery stenosis(OR=-0.051,95%confidence interval:-0.975-(-0.926),P<0.01).4)eGFR sensitivity(81%)was higher than creatinine(4%)relative to sensitivity in determining renal impairment. Conclusion:In patients with acute cerebral infarction, the degree of carotid artery stenosis in those with decreased eGFR was significantly higher than that in those with normal cerebral infarction. The more eGFR decreased, the more severe the renal injury and the higher the degree of carotid artery stenosis. Therefore,it can be seen that low eGFR level may increase the incidence of ACI.In this paper, the predictive value of glomerular filtration rate in the diagnosis of cerebrovascular diseases was further clarified through the study of the relationship between the two.In clinical work,it is convenient to evaluate the vascular status by early use of test indicators, which has certain guiding role in predicting the occurrence, development and prevention of acute cerebral infarction in patients.
