Abstract/Summary:
Objective: Comparative analysis of clinical efficacy of laparoscopic common bile duct exploration(LCBDE)and open common bile duct exploration(OCBDE)in the treatment of common bile duct stones complicated with acute cholangitis.Methods: Through retrospective comparative analysis,we selected patients with common bile duct stones from 2013.1 to 2018.10.According to different surgical treatment methods,they were divided into open group(40 cases)and laparoscopic surgery group(45 cases).The open group underwent traditional OCBDE and the laparoscopic group underwent LCBDE.T tube is placed in both groups.Perioperative data and levels of various surgical evaluation indicators were compared between the two groups.Results: Patients who diagnosed with common bile duct stones with mild to moderate acute cholangitis were all successfully completed the operation 。 The laparoscopic had less operative time(129.27±28.10 min vs 166.23±36.80 min,p =0.000),less surgical blood loss(63.33 ± 36.87 ml VS216.25 ± 105.24 ml,p =0.000)and earlier bowel function recovery(2.12±1.00 day VS 3.3±1.01 day,p=0.044<0.05)than open group.Conclusion:For patients diagnosed with mild to moderate acute cholangitis before surgery,the operation time of laparoscopic common bile duct exploration is relatively shorter than that of open common bile duct exploration.The intraoperative blood loss is less,the visual field is clear and reduced.The operation is difficult,the postoperative ventilation time is earlier,the gastric tube is pulled out early,and the laparoscopic incision is smaller,which is more beautiful than the laparotomy.
