Objective: To compare the prognosis between hepatic resection and transcatheter arterial chemoembolization(TACE)for patients with hepatocellular carcinoma(HCC)with portal vein tumor thrombus(PVTT).Methods: From June 2012 to December 2017,58 patients with HCC and type ?/? PVTT who from Shengjing Hospital were included.According to different treatment,they were divided into hepatic resection group(n=28)and TACE group(n=30).Then compare their overall survival rates and analyse independent risk factors.Survival analysis used Kaplan-Meier method and Log-rank test.Multivariate analysis used Cox proportional hazard regression model.Results: In the hepatic resection group,the median survival time was 13 months,and the0.5-,1-,2-,3-year overall survival rates were 74.7%,54.1%,29.2% and14.6%,respectively.In the TACE group,the median survival time was 6.4 months,and the0.5-,1-,2-,3-year overall survival rates were 53.3%,23.3%,13.3%,8.9%,respectively.The significantly prognostic factors for overall survival in multivariate analyses were multiple tumor(HR:2.578,95%CI[1.365-4.868],P=0.008)and AST>34IU/L(HR:2.509,95%CI[1.266-4.972],P=0.008).Conclusion: Hepatic resection is better than TACE for hepatocellular carcinoma with portal vein tumor thrombus.Multiple tumor and AST>34IU/L were the independent influencing factors for postoperative overall survival. |
