Effects Of Different Fluid Resuscitation Strategies In The Early Stage Of Traumatic Hemorrhagic Shock On TNF-α And IL-6 In Serum Of SD Rats

Objective: To investigate the effects of different kinds of resuscitation solution and different resuscitation methods on tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)of proinflammatory cytokine in serum of SD rats with traumatic hemorrhagic shock(THS)during early fluid resuscitation.Methods:80 adult SD rats were randomly divided into 5 groups,4 experimental groups and 1 control group with 16 rats in each group.The THS rat model was made by femoral trauma method and bleeding method in the 4 experimental groups.The blank control group was treated with venous catheterization and anaesthesia only,and no THS model was established in this group.According to the reagents and methods of resuscitation,the groups were named as 7.5% hypertonic sodium chloride solution restricted fluid resuscitation group(HS),7.5% hypertonic sodium chloride solution and Lactated Ringer’s solution restricted fluid resuscitation group(HS/LR),Lactated Ringer’s solution and hydroxyethyl starch restricted fluid resuscitation Group(LR/HES1),Lactated Ringer’s solution and hydroxyethyl starch sufficient fluid resuscitation group(LR/HES2)and blank control group.After maintaining THS for 1 hour,the rats in the 4 experimental groups began to undergo resuscitation,and the target rats were killed at 2,6,12 and 24 hours after fluid resuscitation,meanwhile,the abdominal aorta blood was taken from each of the 5 groups to terminate the experiment.The levels of TNF-α and IL-6 in isolated serum of 5 groups were measured and compared by enzyme-linked immunosorbent assay kit(ELISA kit).Results:1.The levels of serum TNF-α and IL-6 in 4 experimental groups,namely HS group,HS/LR group,LR/HES1 group and LR/HES2 group,were significantly higher than those in control group(P < 0 05).2.At the 6th hour after resuscitation,the levels of TNF-α and IL-6 in the blood of the four experimental groups reached the peak values in each group,and then decreased at the later time point.3.In the 4 experimental groups,the levels of serum TNF-α and IL-6 in the LR/HES2 adequate fluid resuscitation group were higher than those in the other 3 restricted fluid resuscitation groups at the same time point(P < 0 05).4.The levels of serum TNF-α and IL-6 in HS/LR group were lower than those in HS group and LR/HES1 group at all time points in the 3 groups of restrictive fluid resuscitation group(P < 0 05).Conclusions : 1.In the early stage of traumatic hemorrhagic shock,limited fluid resuscitation was more effective than sufficient fluid resuscitation in reducing the release of proinflammatory factor TNF-α and IL-6 in serum of SD rats.2.The release of TNF-α and IL-6 in the serum of SD rats can be more effectively reduced by the combination of 7.5% hypertonic sodium chloride solution with Lactated Ringer’s solution than the other liquid collocation of two groups of restricted fluid resuscitation.
