Objective: To analyse the retrobulbar ocular blood flow changes after vitreoretinal surgery in patients with primary retinal detachment(PRD).We then explore the influence of silicone filling time and laser photocoagulation of retina to retrobulbar ocular blood flow.Methods: Prospective consecutive study.17 eyes from 17 primary retinal detachment patients scheduled for vitrectomy surgery at ophthalmology department from the forth Affiliated Hospital,China Medical University were recruited.The contralateral eyes were the control group throughout the study.All eyes were examined by a Color Doppler flow imaging(CDFI)at the day before vitrectomy surgery and the seventh day after silicone oil remove.Peak systolic velocity(PSV),end diastolic velocity(EVD)and resistive indexes(RI)were assessed in the ophthalmic artery(OA),central artery(CRA)and posterior ciliary arteries(PCA).At the same time,we observe the condition of retinal reattachment and the recovery of visual founction.Results: PSV of OA and CRA were decreased compare with them(P<0.05).PSV of OA were significantly increased after silicone oil remove,RI were decreased significantly(P<0.05).Postoperative hemodynamic parameters of CRA and PCA were higher than preoperative,but the difference have no statistics significnce(P>0.05).No changes were found in control eyes between preoperation and postoperation.We found a negative correlation between the change of RI in OA and the number of laser point(P<0.05).Moreover,the decreasement of RI in OA and the time of silicone oil filling had positive correlation(P<0.05).Conclusion: VRS might increase the velocity and filling degree of OA,decrease RI and improve ocular blood supply postoperatively.PRP and silicone oil implantation will be helpful to the reattachment of retina,which will contribute to the improvement of retinal blood supply and the recovery of visual founction.
