| Objective: To probe into the correlation between thyroid hormone levels in normal range and the incidence and severity of cardiovascular disease.In order to provide new ideas and more evidence for dealing with the confusions in clinical practice.Methods:Patients who underwent coronary angiography were selected as subjects in the Department of Cardiovascular Medicine of the Heping district of the General Hospital of Northern Theater Command from September 2016 to July2018.According to the coronary angiography,these patients were grouped into non coronary heart disease(non-CHD)and coronary heart disease(CHD).Then the patients with cardiovascular disease were further divided into low score group and high score group according to Gensini score system.On the basis of the number of stenosed coronary vessels,they were divided into single,double,and multi vessel lesions.The demographic characteristics such as age,sex,smoking history,drinking history,diabetes mellitus,hypertension and other past illnesses were recorded and collected.The BMI were calculated at admission and the recent use of drug was also analyzed.Fasting blood was taken from peripheral veins,including thyroid stimulating hormone(TSH),free thyroxine(FT4),free triiodothyronine(FT3),triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),aspartate aminotransferase(AST),alanine aminotransferase(ALT),troponin T(cTn T),creatine kinase isoenzyme(CKMB),serum creatinine(Cr),fibrinogen(FIB),white blood cell(WBC),red blood cell(RBC),hemoglobin(HB),platelet(PLT).The clinical data were analyzed by SPSS24.0 software.Results:1.The age,Gensini score,number of stenosed coronary vessels,incidence of hypertension and diabetes in CHD group were higher than those in non-CHD group,FT3 and HDL were lower than those in non-CHD group.Logistic regression analysis showed that age and diabetes were risk factors in coronary heart disease and HDL was a protective factor.2.There was significant difference in TSH between the two groups with different Gensini scores(P = 0.031).The higher the score,the lower the TSH(r =-0.176,P =0.013).3.A correlation analysis between thyroid hormone and blood lipids showed that there was a negative correlation between FT3 and HDL.4.The correlation between the number of coronary artery lesions and the different index was analyzed.The results showed that the Gensini score was positively correlated with the number of coronary artery lesions(r = 0.614,P < 0.01).TSH was negatively correlated with the number of coronary artery lesions(r =-0.207,P =0.003).5.During the subgroup analysis of coronary artery lesions,there was a significant difference in TSH between single vessel lesions and multiple vessel lesions(P=0.006),the level of TSH was higher in single vessel lesions.Logistic regression analysis showed that TSH decreased with the increase of lesion branches.High level of TSH was a protective factor for coronary heart disease(P = 0.027).Conclusion:The level of thyroid hormone in the normal reference range is not significantly correlated with the occurrence of coronary heart disease,but the concentration of TSH has certain reference value for the severity of coronary artery stenosis in the population with cardiovascular disease. |
