Objective:This study aimed to investigate the prevalence of thyroid disease,thyroid nodules and the Thyroid autoimmune state,the iodine intake in some urban and rural areas in Jiangxi;and to analyze the relationship between iodine intake and thyroid disease.Methods:1.2626 adults were enrolled in this study,including 1,381 urban adults,1,245 rural adults.Serumthyrotropin(TSH),thyroid peroxidase antibodies(TPOAb),thyroglobulin antibodies(TgAb),and urine iodine concentration(UIC)were measured,and all subjects must undergo an ultrasonography of the thyroid.Free thyroxine(fT4)and free triiodothyronine(fT3)levels were only measured if the serum TSH was outside the normal range.2.According to the residence,the subjects were divided into urban group and rural group,and the prevalence of thyroid disease and iodine intake were compared in the two groups.Then,the the prevalence of thyroid diseases and the iodine intake were evaluated in some urban and rural areas in Jiangxi province.3.Logistic regression analysis was used to evaluate the influencing factors of thyroid nodules,abnormal thyroid function,and occurrence of thyroid hormone antibodies.Results:1.The median UIC were 176.5 ug/L in adults ?2.prevalence of clinical hyperthyroidism,subclinical hyperthyroidism,clinical hypothyroidism,subclinical hypothyroidism,and thyroid nodules and thyroid hormone antibody positive rates in Jiangxi Province were 0.7%,0.4%,0.8%,8.9%,and 8.1%,11.3%.3.The binary logistic regression model demonstrated that family history of thyroid disease(OR=2.313,95%CI:1.273-2.424),female(OR=1.069,95%CI:1.051-1.087),and age(OR=0.582).(95% CI: 0.351-0.965)were independent risk factors for thyroid nodules;history of thyroid disease(OR = 3.72,95% CI: 2.249-6.155),female(OR = 0.453,95% CI: 0.292-0.702),Older(p=0.047)and UIC excess(p=0.032)were independent risk factors for thyroid autoimmunity;family history of thyroid(OR=11.163,95%CI:3.346-37.243),male(OR=1.069,95)%CI:1.051-1.087)and excess iodine(OR=0.118,95%CI: 0.027-0.520)were independent risk factors for hyperthyroidism;older(OR=1.013,95%CI=1.005-1.022),female(OR=0.634,95% CI: 0.432-0.931,p=0.02);TPOAb(OR=0.226,95%CI=0.147-0.347),TgAb(OR=0.533,95% CI=0.337-0.842)positive antibody were independent risk factors for hypothyroidism,in addition compared with iodine deficiency,iodine deficiency and iodine excess,iodine excess was independent risk factors for hypothyroidism(OR=0.550,95%CI=0.379-0.797;OR=0.590,95%CI=0.392-0.888;OR=0.192,95% CI=0.059-0.628).Conclusions:1.The iodine nutrition of some urban and rural residents in Jiangxi Province is in a state of adequate iodine;there is no difference in urban and rural areas;female iodine deficiency is more than male;male iodine above adequate and excess are more than female;in olders urine iodine is lower and iodine deficiency is more common;iodine excess is rarer.2.The prevalence of thyroid nodules was 8.1%,hyperthyroidism was 0.7%,subclinical hyperthyroidism was 0.4%,hypothyroidism was 0.8%,subclinical hypothyroidism was 8.9%,and antibody positive was 11.3%.3.Family history,aging,female are risk factors for the occurrence of thyroid nodules.4.Family history,female and aging were associated with antibody positivity.5.Family history,men,iodine excess are risk factors for the occurrence of hyperthyroidism.6.Increasing age,female,antibody positive,iodine excess are risk factors for hypothyroidism.
