| Objectives:l)To explore expression of platelet carcinoembryonic antigen-related cell adhesion molecule-1(CE AC AMI)in patients with acute coronary syndrome(ACS);2)To explore the change and clinical significance of serum CEACAM1 in ACS before and 24h after percutaneous coronary intervention(PCI);3)To explore the relationship between serum CEACAMI and left ventricular remodeling in ACS.Methods:1.From January to December 2018,87 probable ACS patients were hospitalized in the Department of Cardiology,the first affiliated Hospital of Kunming Medical University.Patients were diagnosised by clinical symptoms,electrocardiogram,myocardial necrosis markers andcoronary angiography.Divided into normal coronary angiography(20 cases),unstable angina(20 cases),ST-segment elevation myocardial infarction(25 cases),non-ST-segment elevation myocardial infarction(22 cases).The 4ml blood samples were collected in the next day,stored in refrigerator at 4?,and flow cytometry was performed within 4 hours to detect the mean fluorescence intensity(MFI)value of platelet CEACAM1.2.From January to December 2018,92 probable ACS patients were hospitalized in the Department of Cardiology the first affiliated Hospital of Kunming Medical University.Patients were diagnosised by clinical symptoms,electrocardiogram,myocardial necrosis markers and coronary angiography.Divided into normal coronary artery(24 cases)and ACS(68cases).The 4ml blood samples was collected before and 24 hours after PCI.After centrifugation for 15 minutes,the supernatant was divided into 1.5ml EP tube and stored in refrigerator at-80 ?.The concentration of serum CEACAM1 was detected by xMAP after the samples were collected.3.During hospitalization,blood routine,blood biochemistry,myocardial necrosis markers,B-type natriuretic peptide(BNP)and echocardiography were performed patients.Left ventricular ejection fraction(LVEF),left ventricular end-systolic volume(LVESV)and left ventricular end-diastonc volume(LVEDV)were measured to explore the correlation between LVEF and serum CEACAM1.Results:1.The level of platelet CEACAM1 in ACS group(627.00±530.00)was clearly higher than normal group(351.00±421.00),P=0.003(P<0.05),and the difference was statistically significant;2.The level of platelet CEACAM1 in STEMI group(726.00±735.50)was clearly higher than normal group(351.00±421.00),P=0.001(P<0.05),and the difference was statistically significant.The level of platelet CEACAM1in the UA group(530.50±565.50)and NSTEMI group(507.50±462.25)was higher than normal group(351.00±421.00),but the difference was not statistically significant.Compared with the UA group and NSTEMI group,UA group and STEMI group,STEMI group and NSTEMI group,the P values were 1.000,0.203,and 0.132(P>0.05),and the difference was not statistically significant;3.In STEMI group,the level of platelet CEACAM1 in high thrombus load group was lower than low thrombus load group,but the difference was not statistically significant,P=0.507(P>0.05);4.There was no significant difference among single-vessel disease group,two-vessel disease group and multi-vessel disease group of ACS,but the level of platelet CEACAM1 in double-vessel disease group and multi-vessel disease group was higher than single-vessel disease group;5.The serum CEACAM1 level in the ACS group before PCI was higher than normal group,but the difference was not statistically significant,P=0.100(P>0.05);6.In ACS group,the level of serum CEACAM1 after PCI(24h)was higher than before,but the difference was not statistically significant(P=0.198,P>0.05);7.There was a positive correlation between the level of serum CEACAM1 and LVEDV before and after PCI(24h)in ACS.The correlation test P values were 0.048,0.023(P<0.05).The correlation coefficients were 0.259,0.296,which were mild correlation;but there was no significant correlation between the level of serum CEACAM1 and LVEF,LVESV before and after PCI(24h)in ACS(P>0.05).Conclusions:1.The level of platelet CEACAM1 in ACS group was significantly higher than normal group,and the level of platelet CEACAM1 in STEMI group was significantly higher than normal group;2.The level of serum CEACAMlin ACS was positively correlated with LVEDV before and after PCI(24 h).The innovation of this paper:1.This study was the first to explore the expression of platelet CEACAM1 in ACS patients.It was found that the level of platelet CEACAM1 in ACS patients was clearly higher than the normal coronary artery group,and the level of platelet CEACAM1 in STEMI group was clearly higher than normal group.2.This study was the first to explore the change and clinical significance of serum CEACAM1 in ACS before and after PCI.It was found that the level of serum.CEACAM1in ACS was positively correlated with LVEDV before and after PCI(24h). |
