[Objective]Distal stenosis or occlusion of the lower extremity artery is one of the main pathogenesis of Diabetic Foot(DF),and often combined with proximal lower extremity arteriosclerosis obliterans(ASO).Peripheral vascular intervention and surgical treatment of poor vascular lesions in the distal outflow tract are difficult to achieve satisfactory results.This study used a retrospective analysis to investigate the treatment of diabetic foot with autologous peripheral blood stem cell transplantation on the basis of balloon dilatation and proximal iliac crest,femoral hernia,and iliac artery.[Methods]The Department of Vascular Surgery,the Fourth Affiliated Hospital of Kunming Medical University,collected 96 patients with diabetic foot(Fontaine IV)from October 2016 to January 2018.A total of 116 affected limbs were used to analyze the clinical data of patients with different treatment options.Retrospective analysis.Divided into group A,B,C,group A is the combined treatment group:32 patients(32 limbs),male:female=14:18,age 56 to 67 years,mean age(58.37±5.21)years old According to the results of arterial CTA,the multi-segment vascular lesions were grouped into the sub-abdominal,femoral,sub-abdominal arterial stenosis or occlusion three-point group.Balloon expansion combined with autologous peripheral blood stem cells Transplantation;group B was a simple balloon dilatation group:32 patients(36 limbs),male:female=11:21,age 57 to 71 years,mean age(59.86±6.71)years old,same as group A Arterial CTA group grouping,simple balloon dilatation;Group C:32 patients(42 limbs),male:female=16:16,age 53 to 76 years,mean age(60.03±7.56)years old To control the blood sugar,anti-infection,improve circulation,local debridement and dressing change,as a control group.The basic treatments of group A and group B were the same as those of group C of the control group.The follow-up was performed 3 months and 6 months after discharge.Statistical analysis was used to evaluate the relief of the three groups of subjective indicators,and the objective measures were used to evaluate the effect of intermittent claudication distance,ulcer area,sputum index,transcutaneous oxygen partial pressure and improvement of dorsal artery blood flow[Results]1.There were no significant differences in the cold score,ulcer area score,intermittent claudication,sputum index,percutaneous oxygen partial pressure,and dorsal artery blood flow score between the three groups before treatment(P>0.05).After 3 months and 6 months of treatment,the difference of cold score,intermittent claudication,ulcer area score,sputum index,percutaneous oxygen partial pressure and dorsal artery blood flow score in group A and group B was statistically different from that in group C.Significance(p<0.05),suggesting that both treatment regimens are effective.2.There was no statistical difference between the two experimental groups A and B before the treatment,such as cold score,ulcer area score,intermittent claudication,sputum index,transcutaneous oxygen partial pressure,and dorsal artery blood flow score.Learning significance(p>0.05),after 3 months and 6 months of treatment,the scores of cold,ulcer area,intermittent claudication,sputum index,transcutaneous oxygen partial pressure,and dorsal artery blood flow scores were different.Statistical significance(p<0.05),suggesting that the combined treatment group is better than the simple balloon dilatation group.Third,the balloon expansion combined with the autologous peripheral blood stem cell transplantation group was followed up for 1 year,and no abnormalities such as thrombosis and embolism were found.[Conclusion(s)]Patients with diabetic foot and lower extremity arteriosclerosis obliterans combined with autologous peripheral blood stem cell transplantation on balloon expansion can improve symptoms,improve clinical efficacy,promote ulcer healing,and effectively reduce the rate of cut.
