ObjectiveTo investigate the effect of hypertensive disorder complicating pregnancy on maternal coagulation function,and to study the predictive value of thromboelastography on hypertensive disorder complicating pregnancy,so as to provide more reference for clinical treatment and prevention.MethodsA retrospective analysis of 93 pregnant women with gestational hypertension in our department from Jannuary 2017 to June2018 was conducted as a research group 95 normal pregnant women were used as reference group.Coagulation function test and thromboelastogram(TEG)were performed in both groups of pregnant women.The changes of coagulation function indexes and TEG parameters were analyzed in two groups of pregnant women,and the predictive value of TEG for hypertensive diseases in pregnancy was discussedRESULTS(1)Compared with the control group,the PT,APTT,and TT levels in the study group were lower than the control group,and the level decreased with the severity of the disease;FIB,DD The level was higher,and the level increased more with the severity of the disease,and the comparison between the groups was statistically significant,P<0.05.There was no significant difference in INR levels between the two groups(P>0.05).(2)According to the platelet detection data analysis,compared with the control group,the PLT level of each group in the study group was significantly decreased,and the level decreased with the severity of the disease;while the PDW and MPV levels were higher,and As the severity of the disease,the level increased more,the comparison between the groups was statistically significant,P<0.05.(3)According to the data of thrombus elastic force test,compared with the control group,the RG and K values of the TEG parameters of each group in the study group decreased,and the parameter value decreased with the severity of the disease;Both the angle and the MA increased,and the severity of the disease increased with the severity of the disease.The comparison between the groups was statistically significant(P<0.05).(4)Correlation analysis between coagulation index and platelet index of pregnant women in the study group and TEG parameters.After normality test,APTT,PLT,TT,R,MA were normally distributed(P>0.05),PT,FIB,INR,DD,PDW,MPV,a,and K were non-normal distributions(P<0.05),normal data were analyzed by pearson correlation,and non-normal data were analyzed by spearman correlation.The R value was positively correlated with PT,APTT and TT,r=0.259/0.213/0.301,P<0.05/P<0.01,which was statistically significant.K was positively correlated with APTT,r=0.332,P<0.05,statistically significant.There was a negative correlation with FIB and PLT,r=-0.165/-0.248,P<0.05;statistically significant;? was positively correlated with PLT and FIB,r=0.416/0.512,P<0.01,with statistical significance;MA was positively correlated with PLT and FIB,r=0.597/0.394,P<0.05,which was statistically significant.The remaining indicators were not significantly correlated with the parameters of TEG(5)The sensitivity of TEG parameters and coagulation indicators to hypercoagulant state of hypertension during pregnancy was evaluated by paired four-cell chi-square test(McNemar).The test results suggested that R value was highly sensitive to hypercoagulant state,and alpha and MA were highly sensitive to increased fibrinogen.ConclusionThe maternal blood of hypertensive disorder complicating pregnancy is hypercoagulable,and it is aggravated with the severity of the disease.The use of thrombus thrombus to detect maternal coagulation indicators can help to understand the patient's condition and its severity.The progress and complications of hypertension,as well as providing guidance for clinical prevention and treatment,play an important role.
