| Purpose:collected patients with ventricular tachycardia(VT),ventricular fibrillation(VF),through application of Amiodarone and Niferalant hydrichlonde,evaluation the clinical curative effect and safety of Niferalant hydrichlonde1.Research methods?general information:collect 40 cases with diagnosis of ventricular tachycardia and ventricular fibrillation from 01,2016-04,2017 in department of emergency medicine of our hospital.20 cases treated with Amiodarone,20 cases treated with Niferalant hydrichlonde(12 cases treated with only Niferalant hydrichlonde,3 cases treated with Niferalant hydrichlonde after ineffective treatment of Amiodarone).There were 24 cases of male,16 cases of female,in age 56.15±16.11 years.?the observation indexes:To observe the ECG parameters before and after medication[blood pressure(BP),heart rate,QT interval,after correction of QT interval(QTC)];Time to onset of medication(t);The implementation of the electric compound(DC)(The number of application of DC,ECG parameters);Drug adverse events during the period of abnormal and laboratory examination results[the single-line VT was converted into multi-formVT,VT was converted into VF,the time of appearance the other non-ventricular arrhythmia and Torsade depointes(Tdp)procedures,the correlation between medicine and results].?data analysis:using SPSS17.0 software analysis,normal metering data including the ECG parameters before and after medication(BP,P,QTC)and the onset time(t)with(X)+/-standard deviation(S),the numerical comparison by using paired t test,count data ratio by using x2 test(c/o)rate,inspection standard a=0.05,P<0.05 defined difference was statis-tically significant.2.Results:The institute's collection of 40 cases of patients,20 patients with application of amiodarone(50%),20 patients with application of Niferalant hydrichlonde(50%),in addition to the 9 cases(22.5%)patients without underlying heart disease,the rest all have heart disease patients,including 20 cases(50%)of coronary heart disease and corpulmonale in 1 case(2.5%),idiopathic ventricular tachycardia in 2 cases(5%),cardiomyopathy and 6 cases(15%),congenital heart disease surgery in 1 case(2.5%),valvular disease,1 case(2.5%),there is no basis for heart disease 9 cases(22.5%).From 40 cases,VT18 cases,VF22 cases,for patients with ventricular tachycardia,if the hemodynamic stability,application given intravenous Both,if hemodynamic instability,the use electric defibrillation is invalid after application of all,in the process of application,the load of the amiodarone is 150 mg 3 min intravenous injection,maintain the quantity of 300 mg of trace pump pumping,continuous observation time 24 h;The Niferalant hydrichlonde loading dose of 0.3 mg/kg 5 min intravenous injection,maintenance dose of 0.4 mg/kg at least 1 h intravenous drip,duration observation 6 h,the application of amiodarone 8 cases were markedly improved(40%),effective 8 cases(40%),invalid in 3 patients(15%),deterioration in 1 case(5%).Application,the Niferalant hydrichlonde,11 cases were markedly improved(55%)[used alone Niferalant hydrichlonde,9 cases were markedly improved(45%),application of amiodarone is invalid after application,the Niferalant hydrichlonde,2 cases were markedly improved(10%)],6 cases(30%)[used alone,the Niferalant hydrichlonde in 2 cases(10%),effective application of amiodarone is invalid after application,the Niferalant hydrichlonde effective 4 cases(20%)],ineffective in 2 cases(10%),1 case(5%).Of 40 cases of patients with 1 month of follow-up,application of amiodarone in 20 patients,13 cases died(65%),survival associated with brain dysfunction in 2 cases(10%),vegetative state in 3 patients(15%)and lost to follow-up in 2 cases(10%);Of the 20 patients in the application,14(70%)were killed,and 2(10%)and 4 cases(20%)of the vegetative state were associated with brain dysfunction(10%).Conclusion:This study collected through clinical 40 patients with ventricular tachycardia and ventricular fibrillation,intravenous application of amiodarone and Niferalant hydrichlonde injection,through clinical observation and study the following conclusions:1.The Niferalant hydrichlonde to sustained ventricular tachycardia,ventricular fibrillation can effectively terminated,the Niferalant hydrichlondework more quickly and efficiently than amiodarone,the long-term efficacy of the two drugs has no difference.In contrast to iodide,the incidence of Tdp was not significantly affected by the absence of a significant effect on blood flow dynamics,and the risk of Tdp increased when the two drugs were combined Therefore,in practical application,the QT period should be closely monitored to avoid the occurrence of Tdp.So the clinical efficacy and safety of injection fluid in ventricular and ventricular fibrillation were better in patients with ventricular and ventricular fibrillation. |
