| Objective:To investigate the levels of serum vascular endothelial growth factor(VEGF)and cystatin C(Cys-C)in different stages of type 2 diabetic kidney disease(DKD),and to provide a new direction for the early diagnosis and treatment of DKD.Methods:Eighty-six patients with type 2 diabetes mellitus(T2DM)admitted to the Department of Endocrinology,Affiliated Hospital of Taishan Medical College from January 2017 to October 2017 were selected and divided into 3 groups based on the 24 hour urinary albumin excretion rate(UAER)level:the simple diabetes group(30 patients),the early diabetic kidney disease group(28 cases)and the clinical diabetic kidney disease group(28cases).At the same time,29 healthy subjects from the same period were selected as the control group.The general clinical data such as age,sex,course of disease,systolic blood pressure(SBP),diastolic blood pressure(DBP),and body mass index(BMI)were collected from each group.At the same time,the clinicalindicators such as Cys C,Serumβ2-Microglobulin(β2-MG),creatinine(Scr),urea nitrogen(BUN),fasting blood glucose(FPG),glycated hemoglobin(HbA1c),total cholesterol(TC),triglyceride(TG),High density lipoprotein cholesterol(HDL-C)and low density lipoprotein cholesterol(LDL-C)were collected from each group.And double antibody sandwich enzyme-linked immunosorbent assay(ELISA)was used to determine serum VEGF levels in each group.All data were statistically compared.Results:1.There was no statistically significant difference in gender,age,BMI,course of disease,SBP,DBP,TC,TG and HDL-C between the four groups(P>0.05).2.Compared with the control group,FPG levels were higher in the simple diabetes group and DKD groups,and the difference between the groups was statistically significant(P<0.05);but there was no significant difference in FPG and HbA1c levels among the simple diabetes group and the DKD groups(P>0.05).3.There was no significant difference in Scr and BUN levels among the control group,the simple diabetes group,and the early DKD group(P>0.05);However,the levels of Scr and BUN in the clinical DKD group was significantly higher than the first three groups,and there was a statistically significant difference among the groups(P<0.05).4.In the simple diabetes group,early DKD group and clinical DKD group,24-hour urinary albumin excretion rate(UAER)increased in turn,and there was a statistically significant difference among the groups(P<0.05).5.Compared with the control group,the levels of serumβ2-MG,Cys C and VEGF in the simple diabetes group,early DKD group and clinical DKD group increased in turn,and there was a statistically significant difference among the groups(P<0.05).6.Pearson linear correlation analysis showed that the level of VEGF was positively correlated with the level of Cys C,serumβ2-MG,UAER,Scr and BUN(P<0.05);the level of Cys C was positively correlated with VEGF,serumβ2-MG,UAER,Scr and BUN(P<0.05).Conclusion:1.Serum VEGF and Cys C are closely related to the degree of renal damage in type 2diabetes,which can be used as an important indicator for early screening of type 2 diabetic kidney disease.2.Serum VEGF is related to the progression of type 2 diabetic kidney disease.Rational regulation of VEGF expression in diabetic kidney may be a new direction for the treatment of type 2 diabetic kidney disease. |
