BackgroundMultiple myeloma(MM)is a tumor originating from bone marrow plasma cells,accounting for 10% of malignant hematological malignancies,accounting for 1% of all tumors.So far,it is still incurable.With the prolongation of life and the increase of the population,the incidence of the 50~70 years old population is higher and higher.The etiology of this disease is not clear yet.At present,it is related to heredity,environmental factors,viral infection,ionizing radiation,chronic inflammation,chemicals and antigen stimulation.The clinical manifestations include bone damage,hyper calcemia,anemia,renal dysfunction,infection,and association with high viscosity syndrome syndrome,bleeding tendency,amyloidosis and Raynaud’s phenomenon etc.Studies have shown that APAK is a P53 negative regulatory protein.It mainly occurs in fetal liver,fetal spleen and adult salivary glands,bladder,left ventricle and apex.High expression in a variety of leukemia cell lines and some malignant hematopathy suggests that APAK may be associated with the onset of leukemia.Foreign Studies on colorectal cancer have found that APAK is highly expressed in patients with progressive disease,but the mechanism is not clear.Kruppel(Kruppel-Associated box,KRAB)zinc finger protein(KRAB-containing zinc finger protein,KZNF)is the largest transcription factor family in the human genome.APAK is a member of the zinc finger protein.The APAK protein is a KZNF member of the laboratory found in our country that can play a role in the P53 transcription factor.It can inhibit acetylation of P53,specifically regulate downstream P53 related apoptosis related target gene expression,but has no significant effect on cell cycle related target gene expression.At present,there is less research on APAK protein and multiple myeloma.In order to increase the understanding of the pathogenesis of multiple myeloma,we collected and diagnosed the MM patients,and analyzed the clinical significance of APAK protein expression in multiple myeloma,and explored the correlation between APAK and multiple myeloma.ObjectiveTo investigate the expression of APAK in healthy adults and first diagnosed multiple myeloma(MM),and to make a statistical analysis.The changes of APAK before and after treatment in MM patients were compared and the clinical significance was analyzed with clinical data.Methods1.200 patients in Department of Hematology,Liaocheng People’s Hospital and Blood Diseases Hospital,Chinese Academy of Medical Sciences were selected as the research object.Case control study was adopted to divide the patients into two groups,group A was 100 cases healthy adults,and group B was 100 cases of first diagnosis of MM.At the same time,the peripheral blood of the patient was extracted,RNA was extracted,and the expression of APAK was compared with RT-PCR.At the same time,the peripheral blood protein was extracted and the expression of APAK in different groups was detected by Western-blotting.2.The same chemotherapy regimen was adopted for the first patients to be completely relieved,and the APAK of the same patient was compared before and after treatment.Results1.RT-PCR detection of APAK gene was not expressed in 100 healthy adults,and 95 cases of MM had high expression in 100 cases.2.95 patients took the same PD program(bortezomib plus dexamethasone),and 90 cases of MM patients APAK was down regulated before treatment.ConclusionsAPAK is highly expressed in some MM patients,suggesting that APAK is correlated with the pathogenesis of MM,and it is also related to the progression of disease,but its pathogenesis is not clear yet.
