Objective:Multiple myeloma?MM?is a blood system disease with abnormal cloning and malignant proliferation of plasma cells.The clinical manifestations and first-time symptoms are diverse,and the prognosis of patients is heterogeneous.To explore the significance of neutrophil-lymphocyte ratio?NLR?in the prognosis of multiple myeloma?MM?and to provide evidence for MM prognosis assessment.Methods:Sixty-five patients with initial MM and 83 healthy subjects who were admitted to the Gansu Provincal Hospital of from January 2016 to 2018 were selected as subjects.The clinical data of the patients were summarized by retrospective analysis and calculated according to the results of blood routine examination.NLR values were obtained and the differences between the two values were compared.The median was used as the critical value to classify MM patients into low NLR group?NLR<2.34?and high NLR group?NLR?2.34?.Age,gender,serum calcium?Ca?,serum?2 microglobulin??2-MG?,albumin?ALB?,serum creatinine?Cr?,hemoglobin?Hb?,red blood cell distribution width?RDW?,and lactate dehydrogenase?Differences in LDH?;the above laboratory indicators were grouped to compare the differences in survival rates between every groups,and finally the single factor and COX multivariate regression analysis were used to analyze the relationship between the above clinical indicators and the prognosis of MM patients.Results:1.There was no significant difference in the basic indicators such as gender and age between the case group and the healthy control group,which was comparable?P>0.05?.The NLR level of the case group was higher than that of the healthy control group?z=-2.415,P<0.05?.).2.Patients with high NLR group had higher?2-MG and Cr than low NLR group,but the difference was not statistically significant?P>0.05?.3.Survival analysis showed that the overall survival?OS?of the patients in the high NLR group,the high?2-MG group,the lower NLR group and the low?2-MG group was shorter,and the OS in the high ALB group was lower than the ALB group.The difference was statistically significant.Significance?P<0.05?;the survival rates of patients with age,gender,Ca,Cr,Hb,RDW and LDH were not significantly different,and the difference was not statistically significant?P>0.05?.4.Univariate analysis found that the risk factors affecting the prognosis of MM patients were NLR,?2-MG and ALB.Multivariate analysis showed that NLR and ALB were independent risk factors for the prognosis of MM patients.Conclusion:NLR levels in patients with MM were significantly higher than those in healthy people,and elevated NLR in newly diagnosed MM patients often indicated poor prognosis,which was an independent risk factor for prognosis,although?2-MG and ALB were not independent risk factors for MM prognosis,But they are related to low OS.Therefore,it should also be given high priority.
