| Objective: to analyze the inpatient cost of hypertension patients in a top three hospital in Xinjiang from 2015 to 2019,to master the different social and clinical characteristics of hypertension inpatients;to understand the internal composition of the inpatient cost by using grey relation analysis(GRA)and structural change degree;to explore the influencing factors of the inpatient cost of hypertension patients by using multiple linear regression and BP neural network model.To provide scientific and reasonable basis and suggestions for effectively controlling the increase of hospitalization expenses of hypertension patients and reducing the economic burden of patients.Methods:data were screened and excluded,and 12999 patients were included in the data for retrospective analysis.Taking 2019 as the base year,the hospitalization expenses of other years are discounted by using the consumer price index of health care residents.The new grey correlation method was used to analyze the correlation between the total cost and the cost of hospitalization,and the structural change degree was used to analyze the change of the single cost and the total cost of hospitalization.SPSS 22.0 was used to analyze the hospitalization expenses.Because the hospitalization expenses did not obey the normal distribution,the single factor analysis used nonparametric rank sum test,and the multi factor analysis used multiple linear regression analysis.The BP neural network model of the influencing factors of hospitalization expenses was established and compared with the multiple linear regression analysis.Results: the number of patients with hypertension increased year by year in the past five years.In 2019,the number of discharged patients with hypertension was 1.56 times higher than that in 2015.(1)Basic information of hypertensive patients: among 12999 hypertensive inpatients,most of them are female inpatients,accounting for 57.15% of the total number;most of them are medical insurancepatients,accounting for 58.64%;51-60 years old is the high incidence age of hypertension patients,so the proportion of retired(retired)employees is the largest,accounting for23.26%;(2)basic information of hospitalization expenses of hypertensive patients: 20 From 15 to 2019,the total cost per capita of inpatients with hypertension was 7342.52 yuan,and the total cost per capita fluctuated slightly without significant increase or decrease.In the past five years,the three items that account for the largest proportion of per capita hospitalization expenses are: drug expenses(45.63%),examination expenses(18.11%)and other expenses(13.24%);grey correlation analysis shows that the largest correlation among the composition of hospitalization expenses of hypertension patients is drug expenses,and the lowest is operation expenses.The analysis of structural variability showed that the total structural variability of hospitalization expenses of hypertension patients was 30.91%,and the average structural variability was 7.73%.The major changes in the structure of inpatient medical expenses are inspection expenses,drug expenses and other expenses.(3)The results of univariate analysis were included in the multiple linear regression model.It was found that age,occupation,length of stay,mode of payment,operation,complication and condition of admission were the factors influencing the hospitalization expenses of hypertension patients.(4)By comparing the performance of BP neural network model and multiple linear regression model,it is found that the decision coefficient(R2)of BP neural network model is closer to 1 than that of multiple linear regression model,so the fitting degree of BP neural network model is better than that of multiple linear regression model.Conclusion: the internal composition of hospitalization expenses of hypertension patients is unreasonable,and the proportion of medicine expenses,examination expenses and other expenses is too high.The main influencing factors of hospitalization expenses of hypertension patients are surgery,hospitalization days and complications.Based on the above main factors,more favorable cost control measures should be taken to reduce the hospitalization expenses of hypertension patients. |
