Objects:Through clinical randomized controlled study,we compare the clinical effect and MRI changes of patients with cervical disc hemiation(CDH)treated with Xiaosui Huahe decoction combined with western medicine and Western medicine alone.(2)By analyzing the MRI images and clinical symptoms,the correlation between MRI and clinical symptoms in patients with cervical disc herniation was summarized.(3)The MRI images of the patients were observed to explore the influence of the course of disease,Modic changes,disc degeneration and bull’s eye configuration on the resorption of the herniated nueleus pulposus(HNP)Methods:(1)From January 2018 to June 2019,50 patients with cervical disc herniation who were treated in the department of Suzhou traditional Chinese medicine hospital were divided into two groups.The treatment group was treated with Xiaosui Huahe Decoction and Western medicine,and the control group was treated with western medicine only.Compare the improvement of clinical symptoms of the disease by two different treatment schemes.The evaluation indexes are VAS score,NDI index,SF-36 score,etc.(2)By comparing the MRI images before and after treatment between the treatment group and the control group,the clinical characteristics of the resorption of intervertebral disc herniation in the range of cervical vertebra were analyzed.The evaluation indexes were protrusion rate and absorption rate.Results:(1)The improvement rate of VAS and SF-36 in the two groups was tested by independent t test,and the improvement rate of NDI was tested by Mann Whitney U test(P<0.05).The results were statistically different.The improvement effect of pain,function and quality of life in the treatment group was significantly better than that in the control group.(2)The absorption rate of the treatment group and the control group after the follow-up was tested by independent t test:P<0.05,the results were statistically different.The absorption rate of the treatment group was better than that of the control group,indicating that the treatment group was better than the control group in promoting the absorption of cervical intervertebral disc.(3)The improvement of VAS and absorption rate had no obvious correlation by the Spearman correlation analysis(r0.01=0.137,P=0.344,P>0.05).The improvement of NDIFSF-36 and absorption rate had moderate positive correlation by the Spearman correlation analysis(r0.01=0.454/0.519,P<0.05)(4)Kruskal-Wallis test or Mann Whitney U-test was used to analyze the influence of course of disease,Modic change,and intervertebral disc degeneration on the absorption rate.It was found that there was no statistical difference in the absorption rate between different course of disease and whether there was disc degeneration or not(P>0.05),but there was statistical significance in the distribution difference between Modic changes(P>0.05),cases without Modic changes are more likely to have resorption.Using Spearman correlation coefficient to analyze the correlation between the edge thickness of the bull’s eye configuration on MRI and absorption rate in the treatment group,there is a significant positive correlation between the two(r0.01=0.975,P=0.005),the greater the edge thickness of bulleye sign,the more conducive to the occurrence of reabsorption.Conclusion:(1)Xiaosui Huahe Decoction play a good therapeutic effect on CDH.(2)Xiaosui Huaye Decoction can promote the resorption of some patients with cervical disc herniation.The higher the absorption rate,the better the NDI and SF-36 score.(3)Patients with bull’s eye configuration on MRI and no Modic changes are more likely to have resorption.(4)Course of disease,disc degeneration show no significant effect on resorption of the herniated nueleus pulposus(HNP).
