| Objective: Through the study of perioperative intertrochanteric fracture of femoral bone before and after the implementation of clinical route management of integrated Traditional Chinese and Western medicine to provide reference for further optimization and promotion of the application of clinical route of integrated Traditional Chinese and western medicine.Methods: A total of 93 cases of intertrochanteric femur fractures that met the inclusion criteria were selected from January 2016 to July 2019 in the orthopedics department of Chuzhou hospital of integrated Chinese and western medicine,divided into traditional group(January 2016-December 2017,before the implementation path)41 cases and path group(January 2018-July 2019,after the implementation path)52 cases,not in the same period cohort analysis comparison between the two groups of hospitalization days and total hospitalization expenses and hospitalization complications,improvement of pain during perioperative period,etc.Results:(1)There was no significant difference between the two groups in general information(age,sex,comorbidities,surgical methods),and the two groups were comparable(P > 0.05).(2)In terms of health economics,the total cost of hospitalization,equipment cost,total cost of medicine,cost of Chinese patent medicine,cost of Western medicine,cost of inspection and net cost of hospitalization in the path group were significantly lower than those in the traditional group,while the operation cost and cost of traditional Chinese medicine treatment were significantly higher than those in the traditional group,the difference between the two groups was statistically significant(P < 0.05).(3)In terms of TCM treatment,the participation in TCM characteristic treatment in the path group was significantly higher than that in the traditional group,the difference between the two groups was statistically significant(P < 0.05).(4)In terms of hospitalization days,the average hospital stay in the path group was lower than that in the traditional group(17.52 ±3.89 d vs 17.63 ±4.28d),but there was no significant difference between the two groups(P > 0.05).(5)In terms of medical quality,the path group was significantly lower than the traditional group in terms of complications during hospitalization,and was significantly better than the traditional group in terms of VAS score improvement and the rate of excellent hip function,with statistically significant differences(P < 0.05).Conclusion:(1)After the implementation of clinical pathway management of integrated Chinese and western medicine in the diagnosis and treatment of intertrochanteric fracture,Chuzhou hospital of integrated Chinese and western medicine standardized the diagnosis and treatment behavior,and the net hospitalization cost of the patients decreased significantly compared with the past,which is in line with the current direction of medical reform.(2)Under the intervention of the clinical pathway of integrated traditional Chinese and western medicine,the participation of TCM treatment in the path group was significantly higher than that in the traditional group,which gave better play to the application of TCM characteristics in bone injury diseases.(3)After combine traditional Chinese and western medicine clinical pathway management,path group on perioperative complications was lower than that in traditional group,in pain,and on the hip function improved significantly better than the traditional group,showed that Chinese and western medicine combined with the implementation of clinical pathway effectively improve the medical quality,for the further optimization,improve and promote the development of clinical pathway which combines traditional Chinese medicine with western medicine to provide the basis. |
