Clinical Observation Of Xuefu Zhuyu Decoction Combined With Arthroscopy In Treating Qi Stagnation And Blood Stasis Syndrome Of Knee Osteoarthritis

Objective:To observe the clinical effect of Xuefu Zhuyu decoction combined with arthroscopy in the treatment of knee osteoarthritis and its improvement of TCM symptoms,pain and joint function and its safety,and to explore a treatment method with small trauma,good curative effect quick effect,and can minimize the pain and economic burden of patients.Methods : From November 2017 to October 2019,60 patients with knee osteoarthritis were selected for arthroscopic treatment in the Department of Orthopaedics and Trauma,affiliated Hospital of Shanxi College of traditional Chinese Medicine,and TCM dialectical Qi stagnation and blood stasis syndrome.According to the random number table method,they were divided into control group and treatment group,30 cases in each group.The control group was treated with simple knee arthroscopy;the treatment group was treated with Xuefu Zhuyu decoction on the basis of the control group,and took orally on the second day after operation,1 dose per day,divided in the morning and evening,and 2 weeks as a course of treatment,shared for 4 weeks.To observe the improvement of TCM symptom score,pain and joint function of knee joint before and after treatment in the two groups;the pain and function improvement of knee joint were evaluated by visual analogue pain score and Lysholm function scale before treatment and at the 2nd,4th and 8th weeks after operation;and the related data were statistically analyzed and processed by SPSS19.0after observation.Results:(1)There was no significant difference between the two groups(P >0.05).(2)In the aspect of TCM syndrome: before the treatment,there was no significant difference between the two groups(P > 0.05);after the treatment,the two groups were compared before and after,and the difference was statistically significant(P The treatment group had a significant advantage in improving knee tingling,swelling around the joint and low back and knee tenderness(P < 0.05),but no significant difference in flexion and extension disorders and joint stiffness(P > 0.05).The effective rate of the treatment group was 93.33%,which was slightly better than that of the control group(P < 0.05).(3)Visual analogue pain score: there was nosignificant difference in VAS score between the two groups before the treatment(P >0.05);in the second week after the operation,there was statistical difference in VAS score between the two groups(P < 0.01);in the inter group comparison(P > 0.05),there was no statistical difference,indicating that the two treatment methods could reduce VAS score at the second week after the operation,but the two methods could reduce VAS score There was no significant difference between the two groups;at the4 th and 8th week after operation,the VAS scores of the two groups were compared and found that the VAS scores of the treatment group were significantly lower than that of the control group,and the difference was statistically significant after the rank sum test(P < 0.01).It can be considered that the treatment group was superior to the control group in pain relief.(4)Lysholm joint function: before treatment,the Lysholm joint function scores of the two groups were compared(P > 0.05).There was no significant difference between the two groups,which was comparable.After treatment,the Lysholm scores of the two groups at the 2nd,4th and 8th weeks were compared with those before treatment(P < 0.05),which was statistically significant.It showed that the knee joint function after treatment was better than that before treatment The Lysholm scores of the two groups at the 2nd,4th and 8th week after treatment were compared and found that the average scores of the treatment group were higher than those of the control group(P.(5)Safety: during the observation period,all safety indexes of the two groups were normal before and after treatment,indicating that the two groups were relatively safe.Conclusion:Compared with the treatment of knee osteoarthritis of Qi stagnation and blood stasis,the improvement of Xuefu Zhuyu Decoction combined with knee arthroscopy and knee arthroscopy can improve the symptoms and signs of patients,especially for postoperative knees.Joint function rehabilitation is effective,and it is worthy of clinical promotion and application.
