ObjectiveThe purpose of this study was 1)to clarify the current status of self-management in patients with chronic hepatitis B who received antiviral therapy with nucleoside(acid)analogues;2)to explore the factors that influence the level of self-management in patients with chronic hepatitis B who receive antiviral therapy with nucleoside(acid)analogues.To develop intervention plans,and improve the patients' self management ability,management present situation so as to provide basis to improve.MethodsA total of 336 CHB patients who received antiviral therapy with nucleoside(acid)analogues from January 2018 to July 2018 were selected by convenience sampling.Sample source:department of infection,a third-class hospital of Zhengzhou.In this study,a self-designed general information questionnaire was used to investigate the demographic characteristics and disease-related data of the population.The self-management scale and self-efficacy scale were used to evaluate the level of self-management ability and self-efficacy of patients with chronic hepatitis B.Results t test,variance analysis,correlation analysis and stepwise regression analysis were used to analyze the factors affecting the level of self-management.Results(1)The total score of self-management of CHB patients receiving antiviral treatment with nucleoside(acid)analogues was 4.52±0.44,with the high group of 13.4%,the middle group of 73.44%,and the low group of 13.13%.The self-management ability of the patients was at a medium level.The scores of self-management from high to low were:lifestyle management(4.57 ± 0.51),symptom management(4.56 ± 0.58),psychosocial coping(4.50 ±0.56),disease information management(4.44±0.67).(2)Univariate analysis showed that self-management scores of patients with different gender,age,marital status,place of residence,medical payment method,family per capita monthly income,type of NAs,and main diagnosis were significantly different(P<0.05).(3)Correlation analysis showed that antiviral treatment time and self-efficacy were positively correlated with the total score of self-management and scores of each dimension.(4)SPSS stepwise regression analysis results showed that:1)The influencing factors of the total score of self-management were:marital status,antiviral treatment time,self-efficacy,NAs type and main diagnosis.2)The influencing factors of symptom management were gender,duration of antiviral treatment,self-efficacy,NAs type and main diagnosis.3)The influencing factors of lifestyle management were gender,self-efficacy,econmic status,medical payment mode,NAs type and main diagnosis.4)The influencing factors of psychosocial coping were:marital status,self-efficacy,NAs type,and main diagnosis.5)The influencing factors of disease information management are:age,marital status,residence,self-efficacy,NAs type,and main diagnosis.ConclusionsHBV patients receiving antiviral therapy with nucleoside(acid)analogues have a moderate level of self-management and need to be further improved.Disease information management and psychosocial coping are the weak links of self-management.The common factors affecting the total score of self-management and all dimensions of HBV patients receiving antiviral therapy with nucleoside(acid)analogues were:self-efficacy,NAs type,and main diagnosis.Gender,age,marital status,duration of antiviral therapy,place of residence,per capita monthly household income,medical payment method,and HBeAg can also influence the self-management of patients with CHB who receive antiviral therapy with nucleoside(acid)analogues.
