ObjectiveTo investigate the related factors affecting the prognosis of intracranial ependymoma.MethodThe clinical data of 123 patients with intracranial ependymoma confirmed by pathology in the Department of Neurosurgery of the first affiliated Hospital of Zhengzhou University from January 2013 to December 2017 were analyzed retrospectively.The clinical factors affecting the prognosis of intracranial ependymoma were evaluated by Kaplan-Meier survival analysis and multivariate Cox regression analysis.ResultThe average age of onset in 123 patients was 26 years old(1?77),and there was no significant difference between male and female(male:female=1:1.08).Total resection of tumor was performed in 93 cases and subtotal resection of tumor in 30 cases.Postoperative radiotherapy plus chemotherapy was performed in 31 cases,radiotherapy only in 27 cases,chemotherapy in 21 cases and no adjuvant therapy in 44 cases.All patients were followed up for 2 to 80 months.Recurrence occurred in 49 cases,of which 29 died.The 5-year progression-free survival rate and overall survival rate were 42%and 67%,respectively.Kaplan-Meier survival analysis showed that preoperative KPS score?80,gross total resection,postoperative adjuvant radiotherapy,Ki67 index<10%,and pathological grade ? were associated with better prognosis.The recurrence rate of children under 5 years old was higher,and there was no significant difference in overall survival rate.Multivariate Cox regression analysis showed that the extent of surgical resection,radiotherapy,and pathological grade were independent factors affecting the prognosis of intracranial ependymoma.Conclusion1.Surgical resection with maximum safety range is the most important treatment for intracranial ependymoma,postoperative adjuvant radiotherapy is an important supplementary treatment,and the value of chemotherapy needs to be further studied;2.Patients with better preoperative functional status have longer progression-free survival and overall survival than patients with poor preoperative functional status;3.Children under 5 years old have a higher recurrence rate;4.Pathological grade is an important factor affecting the prognosis of intracranial ependymoma.The higher the pathological grade,the worse the prognosis of the patients.
