Changes in Cognitive Function and Cerebral Perfusion inPatients with Anterior Circulation Macrovascular StenosisObjective: To observe the changes of cognitive function and cerebral perfusion in patients with anterior circulation large vessel stenosisMethods: A total of 19 patients with asymptomatic precirculatory macrovascular stenosis were collected as the case group and underwent CTP and CTA examination.Select the region of interest(ROI)at the subfrontal cortex,subtemporal cortex,and semioval center.Collect 23 patients with no vascular stenosis and no acute cerebrovascular disease with the same age and sex were used as the control group.The patients in the control group and the case group took the cerebral cortical perfusion values of the subcortical temporal cortex,the frontal cortex,and the semi-oval area on both sides.The case group calculated the affected side / the healthy side,and the control group calculated the left / right side.At the same time,the Chinese revised Wechsler Adult Intelligence Scale(WAIS-RC)was used to test cognitive function in both groups.SPSS 25.0 was used for data analysis and processing.The measurement data was expressed as mean ± standard deviation(x ± s).The analysis of measurement data difference between different groups was performed using independent sample T test,and the difference between classification data was used chi-square test.In the test,P <0.05 was considered statistically significant.Results:(1)There were no significant differences in general clinical data(age,gender,education level,hypertension,diabetes,hyperlipidemia,smoking,drinking)between the two groups.(2)There were statistically significant differences between the speech measurement and the operation measurement in the adult's Wechsler Intelligence Scale between the two groups,and the differences were statistically significant(p <0.05).(3)There was a statistically significant difference in cerebral perfusion constants between the two groups of interest(p <0.05).The TTP and MTT of the subfrontal cortex,subtemporal cortex,and semi-oval center of the case group were longer than those of the control group.The CBF and CBV of the subfrontal cortex,subtemporal cortex,and semi-oval center of the case group were lower than those of the control group.group.Conclusions: Patients with asymptomatic precirculatory stenosis have decreased cognitive function and cerebral perfusion.Part ? Effect of Endovascular stent-assistant angioplasty on cognitive function in patients with cerebrovascular stenosisObjective: To investigate the Effect of Endovascular stent-assistant angioplasty on cognitive function in patients with cerebrovascular stenosisMethods: Collect 47 cases of anterior craniocerebral circulation(internal carotid artery / middle cerebral artery)stenosis or plaque due to symptomatic cerebrovascular disease(cerebral infarction,TIA,etc.)or physical examination,and diagnosed as craniocerebral by DSA whole brain angiography Patients with anterior circulation with large stenosis who meet the criteria for stenting.All patients were evaluated for cognitive function using the Chinese Webster Adult Intelligence Scale(WAIS-RC)1 day before surgery,1 week after surgery,and 3 months after surgery.Results:(1)There were no stroke events and no deaths in 47 patients.The NIHSS scores were analyzed at the time of admission,at the time of discharge,and at 3 months after surgery.The analysis found that the NIHSS values before and after admission were significantly different(P <0.05),and gradually decreased with time,from 0.79 ± 1.38 decreased to 0.59 ± 1.06 after discharge;after 3 months,the check was repeated and it decreased to 0.26 ± 0.50(see Table 1).And analysis found that the stenosis rate before surgery was significantly reduced(P <0.05),from(83.04 ± 9.41)% before surgery to(19.79 ± 12.81)% after surgery.(2)Total score of speech scale,total score of operation scale,total scale,speech intelligence quotient,and occupational intelligence quotient 1 week after counting were not significantly different from those before treatment(P> 0.05),and total speech scale 3 months after counting Score,total score of operation scale,full scale,verbal intelligence quotient,and operational IQ were significantly higher than those before treatment(P <0.05).(3)For each sub-item,there was no significant difference between 1 week after surgery and pre-operation(P> 0.05),after 3 months after the test,it was found that the scores of the other sub-items except the vocabulary sub-items were significantly improved compared with those before the operation(P <0.05)Conclusions: Simple endovascular stent formation is safe,and the effect on cognitive function is beneficial in patients with anterior circulation large vessel stenosis.
