Clinical Research On The Application Of Endovascular Stent Angioplasty For Treatment Of Multiple Vascular Stenosis Of Cerebrovascular Disease

Objective: In this study,the influencing factors of the security of using endovascularstent angioplasty to treat intracranial arterial stenosis were analyzed, and the perioperative,postoperative incidence of symptomatic thrombosis and postoperative restenosis rate wereanalyzed based on the differentnumber of stent placement for different types of lesions.Methods: A retrospective analysis was made on the clinical data of patients withintracranial arterial stenosis who received endovascular stent angioplasty from Oct2009toOct2011, so as to analyze the success rate of surgery, perioperative complications andrestenosis rate based on the different number of stent placement, degree of stenosis andspecific conditions. Results: In this study, a total of180patients met the inclusion criteria,including236clear responsibilities related vascular lesions.215stents were placed for118cases of male patients and62cases of female patients who aged27-75years old with anaverage age of64.5years old. The surgical success rate was96.5%, the preoperative andpostoperative average stenosis rates of the patients were70.45%and8.04%respectively. Thelocations of stents: multiple stenosis with two stents placement treatment group had35casesof patients, the multiple stenosis with single stent placement treatment group had67cases ofpatients, the single stenosis with single stent placement treatment group had78cases ofpatients.10cases of patients appeared related perioperative complications, the incidence was5.5%.Symptoms are shown as follows:4cases of intraoperative TIA4(2.2%) and1case ofintraoperative cerebral thrombosis (0.5%),4cases of intraoperative vascular spasm (2.2%),5cases of postoperative fever (2.7%).The differences among the complication rates of thesethree groups were not statistically significant (P>0.05),the occurrences of the complication ofthese three groups should not be considered different. The comparisonsamong thepostoperative1year of symptomatic thrombosis of these three groups:2cases in multiplestenosis with two stents placement treatment group,7cases in multiple stenosis with singlestent placement treatment group,9cases in single stenosis with single stent placementtreatment group. The incidences of these three groups were respectively5.7%,10.4%,11.5%.The differences among the symptomatic thrombosis incidencesof these three groups were notstatistically significant (χ2=2.706,P=0.258>0.05), the occurrences of the complication of these three groups should not be considered different.The comparisons among thepostoperative1year of restenosis rates of these three groups:3cases in the multiple stenosiswith two stents placement treatment group,6cases in the multiple stenosis with single stentplacement treatment group,7cases in the single stenosis with single stent placement treatmentgroup. The incidences of the three groups were8.6%,9.0%,9.0%, respectively. Thedifferences among the incidences of restenosis of these three groups were not statisticallysignificant (χ2=0.005,P=0.997>0.05), the occurrences of restenosis of these three groupsshould not be considered different.Conclusions:1. The security of the multiple stenosis with two stents placement treatment group wasequivalent to the security of the multiple stenosis with single stent placement treatment group.2. The incidence of the postoperative symptomatic thrombosis of the multiple stenosiswith two stents placement treatment group was lower than that of the multiple stenosis withsingle stent placement treatment group and the single stenosis with single stent placementtreatment group.3. The restenosis rate of the multiple stenosis with two stents placement treatment groupwas equivalent to the security of the multiple stenosis with single stent placement treatmentgroup and the single stenosis with single stent placement treatment group.4. There were no significant differences between the intraoperative security, incidence ofcomplications and postoperative1year of restenosis rate of the multiple stenosis with twostents placement treatment group and those of the multiple stenosis with single stentplacement treatment group or those of the single stenosis with single stent placementtreatment group. The incidence of the postoperative symptomatic thrombosis of the multiplestenosis with two stents placement treatment group was slightly lower than that of the controlgroup. The surface multiple stenosis with two stents placement treatment is an effectiveendovascular treatment clinically and has great value for the future development and research.
