Chaihu Shugan San In The Treatment Of Functional Dyspepsia:A Systematic Review And Meta-Analysis

Objective: systematically analyzing the clinical efficacy and safety of Chaihu Shugan San in the treatment of functional dyspepsia by a meta-analysis to provide clinical and experimental research on functional dyspepsia a guide.Methods: Seven databases of CNKI,Wanfang Data,Chongqing Weipu Chinese Science and Technology Periodical Database(VIP),China Biomedical Literature Database(CBM),Pub Med,Embase and Medline(up to February 13,2020)were searched to collect the randomized controlled trials(RCTs)of chaihu shugan san for functional dyspepsia.And two reviewers strictly followed the inclusion and Exclusion criteria to choose literatures,extract data and evaluata the quality.The meta analysis was performed with Rev Man 5.3software provided by the Cochrane collaboration network.Results: A total of 740 relevant literatures were retrieved and 27 randomized controlled clinical studies were enrolled,including2312 patients,of which 698 were in the treatment group of chaihu shugan san and 680 were in the control group;471 patients were in the treatment group of chaihu shugan san VS western medicine and463 patients were in the control group.Included studies generally show a low quality.The meta analysis results show that compared with the control group,chaihu shugan san has a better efficacy in total effective rate(OR=4.40,95%CI[2.78,6.96],P<0.00001),overall symptom scores(WMD=-2.51,95%CI[-2.87,-2.14],P<0.00001)and improving the quality of life(WMD=11.26,95%CI[4.50,18.02],P=0.001),no obvious advantages in terms of the five symptom scores of bloating,early satiety,epigastric pain,belching,and acid-refractory heartburn and improving the half emptying time of gastric fluids.Compared with the control group,chaihu shugan san VS Western medicine for functional dyspepsia has a better efficacy in total effective rate(OR=4.19,95%CI[2.35,7.49],P<0.00001)、overall symptom scores(WMD=-2.80,95%CI[-3.97,-1.64],P<0.00001)and relieving anxiety(WMD=-5.37,95%CI[-8.66,-2.08],P=0.001),improving patients’ motilin(MOT)(WMD=82.07,95%CI[74.08,90.06],P<0.00001)and postprandial leptin levels(WMD=5.70,95%CI[4.73,6.66],P<0.00001),no obvious advantages in terms of improving pre-prandial leptin and corticotropin-releasing hormone(CRH)levels and reliving depression.The adverse effects of the study include gastrointestinal discomfort,dry mouth,dizziness,headache,insomnia,rash,spasm,nervousness,etc.In general,chaihu shugan san is safe for FD.Conclusion: chaihu shugan san for functional dyspepsia shows a better effect,which can improve curative effect,improve symptoms,relieve anxiety,improve quality of life and has a low incidence of adverse reactions.However,the original literatures are all about small sample size and high risk of bias.And because of the insufficiency of outcome indicators such as physical and chemical level test,single symptom score,gastric motility test,relapse rate,quality of life evaluation,anxiety and depression scale,etc.It is recommended to conduct further clinical trials with sufficient sample size,reasonable design,and complete outcome indicators,having standardized diagnostic criteria and curative effect evaluation criteria,appropriate random sequence generation methods,good allocation conceamemt schemes,blind methods,complete outcome reports and follow-up.
