Objective: To explore the effect of rhubarb cream on abdominal pressure to prevent postoperative constipation in patients with mixed hemorrhoids,to speed up postoperative recovery of hemorrhoids,reduce complications and relieve patients’ pain.Method: In this study,a randomized controlled experimental method was used to select selective hemorrhoidal hemorrhoids for mixed hemorrhoids-damp heat injection in the gastrointestinal and anal surgery hospital of the First Affiliated Hospital of Guangxi University of Chinese Medicine from January 2019 to October 2019 Of 64 patients,using random number table method to divide the patients into intervention group and control group 32 cases.Both groups of patients intervened on the basis of routine care.The control group used Dahuang ointment Shenque acupoint application.The intervention group used rhubarb ointment as the medium for abdominal push.The patient’s abdomen was evaluated,selected and positioned on the next day,Followed by umbilical push,lateral push,and ascending colon,transverse colon,descending colon,sigmoid colon,and rectum for 5 minutes,15 minutes in total,once a day for 7 consecutive days of intervention.Then evaluate the normal time of bowel sound recovery,the time of first exhaustion,the time of first bowel movement and postoperative complications,and use the constipation symptoms and efficacy evaluation form,self-rating anxiety scale(SAS),Chinese version of the quality of life of patients with constipationTable(PAC-QOL)evaluates postoperative defecation symptoms and efficacy,anxiety level and quality of life.The obtained data was statistically analyzed using SPSS20.0,and the frequency,percentage,mean±standard deviation,and median(interquartile range)were used for statistical description according to the nature of the data.Two independent sample t tests,paired sample t tests,card Square test,rank sum test and repeated measures analysis of variance for statistical inference.Result:(1)This study finally included 64 patients,including 32 in the intervention group and 32 in the control group.There was no statistically significant difference between the two groups of general data such as gender,age,body mass index,ethnicity,marital status,education,occupation,life and work status(P>0.05).(2)Before the intervention,there were no statistically significant differences in the scores and total scores of constipation,SAS scores,PAC-QOL total scores,and scores of all dimensions in the two groups of patients(P>0.05).(3)After the intervention,the recovery time of bowel sounds,the time of first exhaustion,and the time of first bowel movement of the two groups of patients were better than those of the control group(P<0.01).(4)The scores and total scores of constipation symptoms in the two groups of patients before intervention,two days of intervention and seven days of intervention,after repeated measurement analysis of variance,the results showed that: defecation difficulty score,stool character score and frequency of defecation There were intra-group effects(P<0.01),inter-group effects(P<0.05)and interactive effects(P<0.01)in the scores.The defecation time score,the indefinite swell score,abdominal distension score,and the total score of the two groups Internal effects and interaction effects(P<0.05),there is no interaction effect(P>0.05).(5)The intervention group was superior to the control group in the efficacy index of constipation symptoms and thetotal constipation effectiveness for 2 days and 7 days,and the differences were statistically significant(P<0.05).(6)Postoperative complications in the intervention group were better than those in the control group after 2 days of intervention and 7 days of intervention,and the differences were statistically significant(P<0.05).(7)The comparison of the SAS scores of the two groups of patients before intervention,2 days of intervention and 7 days of intervention showed that there are intra-group effects(P<0.01),inter-group effects(P<0.05)and interaction between anxiety level scores Effect(P<0.01).(8)After 7 days of intervention,the PAC-QOL total score and the scores of each dimension of the two groups of patients showed that the PAC-QOL total score and each dimension of the intervention group were lower than the control group,the difference was statistically significant(P<0.05);Comparison of the total PAC-QOL scores and the scores of each dimension between the two groups of patients before and after intervention showed statistically significant differences(P<0.05).Conclusion:(1)Pressing the abdomen by rhubarb cream can shorten the time for bowel sounds to return to normal.(2)Pressing the abdomen by rhubarb cream can reduce the time of first exhaust and the time of first bowel movement.(3)The method of pushing the abdomen with rhubarb cream can improve the symptoms of constipation in postoperative patients and improve the overall effectiveness of postoperative constipation symptoms.(4)The method of pushing the abdomen with rhubarb cream can effectively improve postoperative complications.(5)Pressing the abdomen by rhubarb cream can significantly reduce the anxiety level of postoperative patients.(6)Pressing the abdomen by rhubarb cream can effectively improve the quality of life of postoperative patients.(7)The rhubarb cream abdominal push method provides medical staff with a new clinical nursing technique for preventingpostoperative constipation of mixed hemorrhoids,which expands the new use of surgical postoperative nursing and is worthy of clinical promotion and application.
