Research background and purpose:Bai Juyi,a famous poet in the Tang Dynasty,once wrote:" If you lose your hair in the morning and in the evening,you might as well lose all your hair,so you won' t have any trouble." It can be seen that people in modern and ancient times are worried about the disease of hair loss.Referring to the relevant literature on the progress of hair loss treatment in the past three years,we can see that the therapeutic effect of western medicine,surgery,laser and other therapy is not ideal.The treatment of alopecia by traditional Chinese medicine has accumulated rich expositions and prescriptions after the development of previous dynasties.The study of ancient literature on hair loss in modern traditional Chinese medicine is either limited to a monograph,or focused on the exertion of a certain theory,or partial to the study of the law of prescription as a whole.Collect the relevant contents of hair loss scattered in the ancient literature,and make a comprehensive analysis of the etiology,pathogenesis,treatment,development and evolution of alopecia,which is helpful to the theoretical development and clinical practice of alopecia.Research contents and methods:The electronic database was used to comprehensively search the literature related to alopecia in the ancient books of traditional Chinese medicine(before 1911).First of all,select the representative works of different dynasties as samples for preliminary reading,record the words such as "hair loss","hair fall","hair fall","hair loss"and "hair loss",and make a simple analysis to further determine the search keywords.Secondly,the use of traditional Chinese medicine large-scale electronic series of books the fifth edition of "Chinese Medical Code" for retrieval,in order to ensure the comprehensiveness of the search books,make full use of other database websites,such as traditional Chinese medicine,Sinology masters and so on for supplementary retrieval.Finally,the retrieved provisions are consulted in paper books to ensure that there will be no major errors in the original intent of the provisions.After collecting the data,taking the dynasty as the outline,the collected data are divided into four historical development stages:pre-Qin and Han dynasties,Sui and Tang dynasties,Song,Jin and Yuan dynasties,Ming and Qing dynasties,using traditional philological research methods and descriptive research methods.summarize the etiology,pathogenesis,treatment,medicine and innovation of hair loss in each stage,and analyze the characteristics of hair loss development and the relationship of academic origin and inheritance in each dynasty according to the background of each stage.The text is divided into three parts:The first part describes the definition of alopecia,distinguishes physiological alopecia from pathological alopecia,and introduces the clinical manifestations of seborrheic alopecia and alopecia areata.This paper combs the origin and meaning of the diseases and names related to alopecia in the ancient literature.The second part introduces the related contents of hair loss in ancient literature in four historical periods.The theories related to hair in Huang Di Nei Jing and Nan Jing during the pre-Qin and Han dynasties are the basis for the development of hair loss theory in later generations.The hair medicine recorded in the Classic of Shennong Materia Medica and the record of famous doctors has become a common drug for the treatment of alopecia in later generations.The Treatise on typhoid and miscellaneous diseases in the treatment of hair loss due to fatigue pays attention to reconciling Ying Wei,warming and tonifying spleen and yang,which has an impact on the use of drugs by doctors such as Li Zhongzi and Huang Yuanyu.During the Jin and Tang dynasties,the etiology and pathogenesis of hair disease was comprehensively discussed for the first time,and a large number of external prescriptions for alopecia were preserved in "Qianjinyao prescription","Qianjinyi prescription" and "Outer Taiwan Secrets".However,because of the prescription for disease differentiation,the unclear prescription for syndrome differentiation,and the lack of attention of doctors as a damaging disease,these prescriptions were gradually ignored after the rise of syndrome differentiation and treatment in the Song,Jin and Yuan dynasties.In the early Song Dynasty,there was still a lot of space in Taiping Sheng Hui Fang and Sheng Ji Zong Lu,but there was no such practice in Taiping Huimin he Pharmaceutical Bureau Fang,which was returned by Bo.There were only a few internal pills to treat women's hair loss.The Jin and Yuan dynasties was the beginning of the treatment of alopecia based on syndrome differentiation.Doctors were no longer keen to collect a large number of external prescriptions for alopecia,followed by the internal prescription for the treatment of alopecia.Before the Song,Jin and Yuan dynasties,the overall etiology and pathogenesis of alopecia were mainly blood deficiency and external wind,while the four schools of Jin and Yuan innovated the contents of the etiology and pathogenesis of hair loss under the framework of their own ideological characteristics.Liu Wansu's theory of creating Xuanfu,Zhang Congzheng' s theory of blood-heat and phlegm-dampness alopecia changed the etiology and pathogenesis of alopecia from deficiency to reality,Li Kui treated hair loss from spleen and stomach,Zhu Danxi thought that alopecia was yin deficiency and heat;on the basis of the influence of syndrome differentiation and treatment of alopecia in Song,Jin and Yuan dynasties,the syndrome differentiation and treatment of hair loss in the Ming and Qing dynasties was further developed and perfected,and the "oil wind" and "hair decay" related to hair loss appeared for the first time in the medical books of the Ming and Qing dynasties.On the basis of fully studying the academic thoughts of their predecessors,doctors have made many innovations in syndrome differentiation,treatment,and prescriptions,such as Huang Yuanyu on the basis of the theory of Ying Wei in the Internal Classic of Huangdi and the disharmony between Ying Wei and Ying Wei in the Theory of typhoid and Miscellaneous Diseases.Gong Tingxian and Li Zhongzi paid attention to the treatment of alopecia with spleen and kidney on the basis of nourishing Yingwei with Guizhi decoction as the base.On the basis of paying attention to spleen and stomach,Gong Tingxian and Li Zhongzi paid attention to the treatment of alopecia with spleen and kidney.Li Jing's introduction to Medicine clearly proposed internal wind alopecia for the first time,which made the pathogenesis of wind hair loss shift from focusing on external wind to paying attention to internal wind before the Song,Jin and Yuan dynasties.The third part:combined with the above analysis of the ancient literature of alopecia,it is discussed from three aspects:the evolution process of disease differentiation and syndrome differentiation of alopecia,the similarities and differences of etiology and pathogenesis of alopecia in ancient and modern times,and the characteristics of external prescriptions for alopecia.From the pre-Qin Dynasty to the Qing Dynasty,alopecia experienced the evolution from disease differentiation to syndrome differentiation.Modern treatment based on syndrome differentiation is mainly based on syndrome differentiation,but it can not be ignored that ancient physicians accumulated a large number of general prescriptions in the period of disease differentiation.The combination of syndrome differentiation and disease differentiation may help to improve the clinical curative effect;the etiology and pathogenesis of alopecia in ancient and modern times are inseparable from the five internal organs,qi and blood,and external evil,but on the whole,the ancient treatment of hair loss is heavy deficiency and good use of tonifying drugs.nowadays,the etiology and pathogenesis of alopecia are mainly composed of dampness and heat,yin deficiency and fire,so drugs should no longer focus on warming and tonifying.Based on the analysis of the external prescription for the treatment of alopecia in the Tang and Song dynasties,it is found that it is commonly used to remove the surface,which has the effects of starting the skin,dispelling wind and dispelling dampness,which is helpful to improve the curative effect of the external prescription.Conclusion:1.The pre-Qin and Han dynasties was the foundation period of hair loss theory.The relevant theoretical contents of alopecia in the Classic of Medicine,Materia Medica and Classical prescriptions in this period laid the foundation for the treatment of alopecia by doctors of later generations.2.The Sui and Tang dynasties is the further fulfillment period of the hair loss theory.The discussion on the etiology and pathogenesis of alopecia is further enriched and systematized,but it is still dominated by deficiency.This period recorded many external prescriptions for the treatment of alopecia,and the prescription for alopecia and medical theory developed independently.3.The Song,Jin and Yuan dynasties was the reform period of the theory of alopecia.In the early Song Dynasty,we can see the decline process of the external treatment of alopecia,the internal treatment of alopecia began to rise in the late Song Dynasty,the etiology and pathogenesis of alopecia showed diversified development,paying attention to the real pathogenesis.There is an internal prescription corresponding to the syndrome type,and the prescription for alopecia tends to be integrated with medical theory.4.The Ming and Qing dynasties is the period of inheritance and innovation of alopecia theory.Doctors in the Ming and Qing dynasties inherited the ideas of their predecessors in the prescription of alopecia,and combined with their own understanding,updated the ancient theory and the new use of the ancient prescription.This period is mainly internal treatment of alopecia,but also internal and external treatment.
