Treatment And Risk Factors For Cesarean Scar Pregnancy: A Retrospective Study

Objective Investigate cesarean scar pregnancy risk and protective factors,provide the basis for prevention,and evaluation of laparoscopic uterine artery temporary blocking technique + under hysteroscopy in scar pregnancy electricity cut method of the safety and feasibility.Methods Using logistic regression in cesarean scar pregnancy factors,single factor and multiple factors analysis method,to choose between January 2015 and December 2019 in our hospital treated 98 cases patients with CSP for group,at the same time of scar uterus intrauterine pregnancy 265 cases as control group,normal analysis age,number,number of induced abortion,breech of cesarean delivery cesarean delivery,cesarean delivery time,at the end of the uterus suture way and the relationship between cesarean scar pregnancy.98 cases of CSP in 38 patients with endogenous type CSP,according to different treatment of surgical procedure is divided into two groups.In group A were treated by laparoscopic uterine artery temporary blocking technique + under hysteroscopy in scar pregnancy electricity cut method,group B were treated by uterine artery embolization(UAE)+ under hysteroscopy in scar pregnancy electricity cut method,comparing two groups of operation time,intraoperative blood loss,hospital stay,hospital expenses,serum beta HCG returned to normal time,postoperative period back to normal time.Results Through the single factor logistic regression analysis,a cesarean section scar pregnancy with cesarean delivery times,at the end of the number of induced abortion,breech cesarean section,cesarean delivery time and the way of uterus suture factors have a significant relationship.CSP multiariable logistic regression analysis results show that the number of cesarean delivery,abortion,breech cesarean delivery OR value is greater than 1,the last cesarean delivery time,uterine suture way OR value is less than 1,thedifference was statistically significant(P < 0.05).Bleeding amount of two groups of beta HCG returned to normal time,postoperative serum menstruation normal time difference was not statistically significant,length of hospital stay and hospitalization expenses of group A is lower than group B,the difference was statistically significant,but the operation time of group A B group,the difference was statistically significant.Conclusion Age of CSP has no significant influence,next time conception from previous cesarean delivery time is more than 2 years and adopts double uterine suture method can reduce the incidence of CSP,the number of abortions,cesarean delivery times and breech cesarean delivery is at high risk for CSP.Laparoscopic uterine artery temporary blocking technique + under hysteroscopy in scar pregnancy electricity cut method for treatment of ingrown CSP has the certain safety and success rate,hospitalization time and hospitalization costs can be reduced,fewer complications,has certain therapeutic value in clinic.
