Objective:To test the load of human papillomavirus(HPV-DNA)in high risk-type infection in accordance to the second-generation hybridization capture technology(HC2).The relationship between HR-HPV viral load and cervical lesions in different degrees was analyzed,with the value of HR-HPV viral load test in early diagnosis of cervical lesions and disease prediction being discussed.Methods:Selection was made among the patients who were hospitalized in the gynecological clinic from January 2015 to June 2018 through the retrospective analysis method to screen for cases with cervical cancer HR-HPV positive indication by the second-generation hybridization capture(HC-2)technique.Then the patients showing colposcopic indication were examined by colposcopy,and cervical pathological biopsy was conducted to determine the degree of cervical lesion.Based on the pathological results,the patients were divided into:Normal group(125 cases),CIN ? group(51 cases),CIN ? group(23 cases),CIN ? group(46 cases),cervical cancer group(20 cases);and based on the viral load,they were divided into low viral load group(1.00-99.99),medium viral load group(100-99 9.99)and High viral load group(? 1000.00).The relationship between HR-HPV load and severity of cervical lesions as well as between same and patient's age were analyzed.Data Processing and Statistical Analysis were conducted by using SPSS 24.0.Results:1.In this group of data,11422 patients had undergone the screening for HC2 cervical cancer,with 1542 were found HPV positive.The HPV infection rate was as follows:13.5%;other than that.385 patients underwent the colposcopy and 265 patients experienced cervical biopsy under colposcopy,with the detection rate of cervical lesions:17.19%,and colposcopy biopsy rate of lower cervix:68.8%?2.The relationship between viral load and cervical lesions2.1 when the viral load of different lesion groups was compared with that of the normal group,i.e.,when CIN group I,CIN group ?,CIN group ? and eervical cancer group was compared with the normal group,the difference was found of statistical significance(P<0.05);and there was no significant difference between the CIN group I,CIN group ?,CIN group ? and the cervical cancer group(P>0.05),that is to say,it does not imply that the viral load of cervical cancer group was higher than that of the CIN group.The viral load of HR-HPV in CIN lesion and cervical cancer group was both significantly higher than that in normal control group.2.2 In the normal group and the CINI group,the proportion of low,medium and high in terms of viral load was 69.6%,28%,2.4%and 47.1%,37.2%and 15.7%,respectively.In the CIN? group,the proportion of low,medium and high viral load was 43.5%,34.8%and 21.7%,respectively.This indicated that the possibility of cervical low-grade lesions was higher when viral load was in the low and medium viral load range,and there were statistical differences among different groups.P<0.05.In CIN ? and cervical cancer,the viral load of the low,medium and high viral load groups was 34.8%,43.5%,21.8%and 35%,35%and 30%,respectively.There was no significant difference among the groups(P>0.05),that is,in the CIN? group and the cervical cancer group there was no difference among the viral load groups.When the viral load is in the range of low and medium range,the possibility of low-grade cervical lesions is higher.2.3 The internal relationship between cervical lesions and viral load at different levels was analyzed and compared by taking the low viral load group a control.In comparison of the CIN ?group with control group,the risk of CIN ? cervical lesion was 1.968 times that of non-CIN ? cervical lesion when the viral load was 100.00-999.99.When the viral load was>1000.00,the risk of CIN ? cervical lesion was 9.667 times that of non CIN ? cervical lesion.Compared to the control group,the risk of eervical cancer was 29 times higher than the normal group when the viral load was? 1000.00,which indicated that the higher the viral load,the higher the grade of cervical disease.The OR values of CIN ?-? group and cervical cancer group increased along with the increase of HPV?DNA load.In the high viral load group,the OR values of CIN ?-? group and cervical cancer group increased gradually,which were 9.667,14.500,18.316 and 29.000 respectively.In the high viral load group,the risk of high-level lesions is increased in a gradual pattern,with the higher the HPV viral load in the lesion cervix,the higher the risk of high-level cervical lesions.3.The relationship between the viral load and age3.1 The viral load of each age group was described by mean value and standard deviation,and then the viral load of each age group was compared with that of<35 years old group.There was significant difference in viral load between 35-45 years old group and<35 years old group(P=0.014).There was significant difference in viral load between the age group?45 years old)and the age group<35 years old(P=0.06).It can be observed that patients over 35 years old has an HPV-DNA Viral load higher than<35 year old patients.3.2 There was no significant difference in the distribution of viral load among different age groups(P=0.156).In the low viral load group,the proportions of<35 years old,35-45 years old and?45 years old were 60.21%,31.19%and 8.80%,while in the medium viral load group,the proportions of each age group were 52.6%,30.779%and 16.67%.In the medium viral load group,the proportions of each age group were 50%,38.30%and 11.70%,which shows that the younger patients were more likely to have low viral load of HPV-DNA.Conclusion:1.In this group of data?the HPV infection rate was as follows:13.5%;the colposcopy biopsy rate of lower cervix:68.8%,and the detection rate of cervical lesions:17.19%?2.The viral load of HR-HPV in CIN lesion and cervical cancer group was both significantly higher than that in normal control group.3.Cervical lesions are closely related to high-risk HPV infection.In the lesion of cervical tissue,the higher the viral load of HR-HPV,the higher incidence of cervical lesions with significant increase.4.HPV viral load was associated with agelt can be observed that patients over 35 years old has an HPV-DNA Viral load higher than<35 year old patients.5.Its high viral load can improve the detection of high-level cervical lesions and with predictive effect on the development of cervical lesions,and hence playing a positive role in reducing the incidence of cervical cancer.
